Worker was exiting the cooler unit and as EE was climbing out of the unit, EE hit a metal anchor that was on the wall near the exit with EE's left leg. When EE hit the anchor with EE's leg EE received a laceration.

Showing all 11The employee was simply walking through the Maintenance Shop holding a box cutter they had just got out of the vending machine. Upon inspection of the box cutter, they noticed a foreign object (possibly glue) on the blade. They proceeded to wipe the blade off on their left sleeve, when they cut/punctured their wrist area.
EE was working on a machine when a cylinder press that was suspended in the air swung towards EE and pinched EE's right middle finger against the side of the buckets machine.
Carrying empty container of regent acid, stepped down and out of shed, landed on ankle incorrectly.
Employee tripped over object on floor, fell against cylinders and attempted to catch themselves.
Employee notified us 1 month after last day worked that it may be work related. Company received letter of determination on 11/15/24.
Someone had previously overloaded the conveyance, causing it to fault out. 3 total contractors were trapped, for approximately 3.5 hours. Communication was quickly established and maintained.
EE collapsed in the office prior to starting work. EE was diagnosed with a heart condition.
EE was lifting a 30 lb. bucket from ground level to the back of EE's service truck, when EE felt a sudden pain in left, lower back.
The EE was attempting to get an air slide flowing at the truck load out. As EE was in process of opening a door to check for flow, material spewed out and got in both of EE's eyes. EE went home on EE's own accord a bit later. Once home, EE talked to EE's wife, and decided to go to the Doctor, where EE was prescribed pain medication.
Employee was getting into the cab of the crane. Lost EE's grip with the hand on the rail and fell backwards on to the ground. Hitting with EE's right foot first and then landed on EE's back
