Employee was discarding sand samples into a 5 gallon bucket for disposal back into a sand pile. The employee was in the process of throwing the discarded sand back into the trash pile when they felt a pop in their leg area. Employee was sent to clinic for eval. First aid pulled muscle was diagnosed. Went back 1/14/26 for eval and has hernia.

Showing all 5On 2/25/25, Vista employee was cleaning material buildup under conveyor with a dingo. The employee caught EE's right hand between the handle of the dingo and the conveyor resulting in lacerations. The employee was taken to Granbury Medical Center where EE received stitches. The employee was released the same day able to return to work.
Employee was assisting with a conveyor belt repair. Employees attached belt clamp to the belt and attached a rope on one end and other end attached to a SkyTrak forklift. Employees began to pull belt and did not know the back stop break was engaged. Rope snapped and struck employee in the face cutting EE's lip.
This Incident occurred with a customer. The injured person was not a Vista Minerals employee. A truck driver for Blackhawk Trucking jumped down off of the trailer approximately 5-6 feet after untangling the tarp. This resulted in breaking both ankles.
Vista employee notified Josey's Contractors v-belts were burning off of exhaust fan, later was shut down. Vista called Josey's to replace v-belts. After LOTO, while twisting and turning the belt into place when left handed index finger got caught between belt and equipment.
