Showing all 19Employee was unhooking a generator trailer and using a tool to release the fifth wheel plate. Tool slipped off fifth wheel plate and employee feel backwards on to the ground hitting their head.
Miner was installing water pipe and had the 8 inch plastic pipe on the skid loader forks when the pipe fell off the forks and struck the miner in the right knee causing some swelling and soreness.
Knee locked up while working on a conveyor belt. Foot caught belt or roller and bent leg and knee locked up.

Shop Mechanic was working on Mine equipment and during an installation of a wedged plate it slipped into place and came down on his right thumb.
THE WASH PLANT OPERATOR WAS CLEARING FROZEN MATERIAL OFF A ROLLER GANG WITH A HAMMER WHEN HE SLIPPED AND THE HAMMER BECAME CAUGHT IN THE ROLLER. THE HAMMER AND THE OPERATORS RIGHT HAND WERE PINCHED CAUSING ABRASIONS AND CUTS TO THE RIGHT HAND AND THUMB.
The miner was working on a walkway greasing the plant and stepped backwards into a small opening next to the motor and lost his balance. He fell directly onto the motor which had a eye bolt sticking out of it. The miner fell only backwards landing on the motor and eye bolt.
MINER WAS WORKING AROUND THE PLANT AND WALKED BEHIND PARKED SERVICE TRUCK AND HIT HIS LEFT KNEE ON THE TRAILER HITCH OF THE SERVICE TRUCK.

TRYING TO FLIP 50' OF 4" WATER DISCHARGE HOSE. DAMAGED ROTATOR CUFF IN LEFT SHOULDER. SURGER WAS DONE 7-3-02.
