PINNING A SURGE BIN TO THE LOADER BUCKET. MOVED BUCKET TO GET PIN IN HOLE AND TONGUE OF SURGE BIN CAME FORWARD AND PINNED LEFT FOOT AGAINST LOADER BUCKET, BREAKING TWO BONES IN FOOT.

SHUT DOWN CRUSHER AND WERE GOING TO BEGIN TEARING DOWN FOR A MOVE THE NEXT MORNING. HE OPENED THE ELECTRICAL BOX ON THE PRIMARY PLANT AND WAS GOING TO BEGIN UNHOOKING THE ELECTRICAL CONNECTION S BUT THE BOX WAS STILL LIVE. HE WAS THROWN FROM THE BOX, BUT SUSTAINED SEVERE BURNS TO LEFT HAND AND ARM.
