EE was using a 4 inch knife to open package of tools when the knife slipped, striking EE's left hand.
EE was exiting work truck. When EE stepped onto ground, EE felt knee pop and give out.
EE was lifting oak block to block up crane when a large splinter lodged into left hand on palm below index finger. EE removed what they thought was the entire splinter. Next day, their hand had become swollen, red and painful.
EE received annual hearing test which noted Standard Threshold shift in right ear. EE is a plant operator who operates primary plant from control booth. EE is being scheduled for another hearing test with medical provider.
EE was grinding metal parts in garage. When completed, EE was opening/lifting garage door when EE felt immediate pain in left shoulder.
EE was installing new set of 48" idlers on conveyor. Bar in the idler slipped and came down and caught EE's thumb between idler and steel frame of conveyor.
Employee was removing last bolt holding hub assembly on right rear of loader. Blocks holding oscillating axle shifted causing axle to kick upward crushing pointer and middle finger on right hand between 3/4" air impact gun and frame of loader.
Employee was using a large bar to pry a rock out of a conveyor. While doing so employee's right hand struck a sharp object causing a contusion and laceration.
EE suffered a strain to the right shoulder while driving the truck on rough terrain.
Removing broken drill steel from machine. Each steel weighs about 100 lbs. Loaded 3 steel drills on truck. Felt pain by end of day.
Taking roll crusher frame apart, removing plates, loosened bolt came off pinching finger between socket and plate.
Fall from height. Fractured ribs, abrasions and hurt back.
Employee was disconnecting lifting equipment from a load that was sitting on blocks, cable and shackle were slack, blocks kicked out from under the load, causing the cable to tighten, catching the left ring finger betweeen shackle and cable.
CLIMBING ONTO DRILL STRUCK SHIN ON RUNG.
EE WAS CLIMBING LADDER TO DRILL WHEN SLIPPED AND FELL BACKWARDS OFF LADDER.
EMPLOYEE WAS CLEANING THE WINDOWS ON THE EUC WHEN HE LOST HIS BALANCE AND JUMPED.
