Showing 50 of 93While stockpiling with a front-end loader, miner felt miner pain in lower back. Towards end of shift, pain had worsened. By next morning, pain had increased causing miner to go to ER.
Miner accessed nose of drop deck trailer and fell ~4 feet to ground below and fractured left wrist.
Was stepping off the track of the excavator onto step hearing a pop in their knee and fell.
Removing a bolt with an impactor to replace the wear plate the carriage bolt moved up. Employee pushed down on the bolt and the sharp edge on the bolt caught their glove pulling a finger down on the rotating bolt cutting their finger.
Employee was hit by an anvil from a VSI crusher. The anvils were being removed and thrown out of the machine by another employee. The employee that was struck walked into the area where the anvils were being tossed to the ground. EE was struck in the lower back and buttocks. EE had scrapes and bruising.
Dragging cords feeling pop in lower back. Tried first aid when injury occurred. First visit to see a Dr. was on 8/7/23 with no additional treatment. 8/14/23 for a recheck appointment receiving additional treatment making injury reportable.
Employee was running a scraper and was being pushed by another scraper. EE alleges that the other scraper hit EE hard from behind and EE complained that EE's back was sore and ask to go home for the day. EE supposed to return to work the next day but asked for the day off. On 6/16/23 EE went to the Occ Health Clinic without notifying anyone.
Employee was loading conveyor onto the transport truck grabbing the railing when EE's hand slipped falling straight backwards onto EE's left side.
Employee was cutting conveyor belt when the knife slipped cutting thumb
Miners finger tip was mashed when caught between a drive belt and a pulley when trying to change the drive belts.
Employee was placing angle iron onto a conveyor when EE caught finger between the metal. EE received a laceration to the tip of the finger that required two stitches.
Loader operator was pulling material from a wall and part of the wall fell on the bucket lifting the back end of the loader to buck up and back down. Loader operator reported hitting their head on the headrest.
EE was hitting a tooth on the loader bucket with a maul and a piece of metal came off going into EE's upper leg.
Employee was cutting conveyor belt rubber with a utility knife and the knife slipped cutting EE's thigh.
EE was on the dozer and closed the door of the master switch and grabbed the door handle and EE's hand slipped off the door handle and EE fell. As EE fell EE's right leg hit the track of the dozer.
Employee was lifting the cover on the hydraulic tank and a finger got caught fracturing the pinky finger.
Miner was attempting to unplug a jaw crusher and was struck in the head by a 5 ft bar. EE received 10 staples to close a laceration on EE's head.
The employee was moving 480 volt power cords during plant set-up and strained left bicep. 1st aid treatment at that time. On 9/10/2017 pain was getting worse, ee was taken back to the doctor where ee received lifting restrictions and set-up with physical therapy appointments.
Operating loader and hit a pot hole bruising employee's sciatic nerve.
Employee was changing screens on the Wash Plant. Lumbar strain injury.
Employee was pushing the door release on the excavator and the door release knob broke and a sharp piece cut hand.
Employee was placing a chain around a rock that was stuck in a Jaw crusher. The rock shifted and pinched his hand between the rock and the sidewall of the crusher. He fractured a bone on his left hand.
Employee was lifting power cords overhead and felt a "pop" in his neck and shoulder. He was taken to our doctor who referred him to a chiropractor. The chiropractor adjusted his neck and shoulder.
Injured employee was removing bolts from a belt frame on a crusher screen. He had his arm underneath the frame piece when the final bolt was removed the frame piece fell pinching his arm between the piece and the box. He went to the emergency room and was given a prescription and released back to full duty.
Employee was dragging the fuel hose to the loader to fuel it up. When dragging the hose back to the tank he felt a pop in his hip.
Fell 15 feet to the ground while on the cat walk of jaw crusher directing backhoe operator.
Injured employee tripped in a low spot caused by build up on a catwalk and sprained his ankle.
While servicing the cone crusher, operator's left finger tip was crushed when keeper ring fell while being removed. Medical treatment included xray, antibiotic and pain med, and dressing of wound. Distal finger bone is broken and nail was removed.
Miner received steam burns to his right arm while attempting to service a pump. The pump had lost its prime and had run dry for a period of time building up heat and steam. Miner attempted to open the filler plug and did not notice pump body was hot, and when he unscrewed the plug the steam escaped burning his arm.
Injury was not reported until 9/6/13. EE alleges that he felt a pull in his stomach while adjusting the under belt on the twin screen. He has been diagnosed with a hernia and released back to full duty but will require surgery at a later date. The accident date is a just a guess by the EE.
Employee reached under rock kicker while it was being adjusted, jack slipped, crushing his hand and wrist. Hairline fracture of a wrist bone, medical treatment provided splint and pain medication
dust and metal grindings blew into eye of miner while repairing conveyor belt. Miner was wearing safety glasses and face shield, but should have worn goggles due to windy conditions, and postioned himself upwind of the work area.
While mothballing wash plant for winter, miner slipped on ice next to a light plant, falling onto the hitch of the light plant. Was examined by medical provider and given pain medication
Employee had been cutting a pipe with a band-saw so it would fit in the dumpster. A piece of pipe was on the rack where it had been cut off and employee said when he walked by the pipe it rolled and fell off poking his arm on the way down.
Employee was placing removable legs on a transfer conveyor. Conveyor was held in place by a loader. Removable leg fell and struck employee in the face, fatally injuring the employee.
Handing tools to his partner from the hopper of the conveyor while working on belt. EE turned around and fell(morning dew made the belt slippery) catching his left arm on the hook of a bungee cord.
4000 cone was packed tight with crused rock. EE entered from underneath cone to free material, when material came down on top of EE. He was smothered and confined for one hour til he was released.
Employee was walking up a ladder on a cone crusher, as he was climbing, he looked up as a 1 1/2" diameter rock fell and hit him in the eye. He scratched his cornea, was given eye drops and returned to work.
Working on conveyor wipe bar trying to loosen bolts and nuts when wrench slipped off nut hitting his head above left eye.
Tightening head pulley scraper on #2 sand conveyor with bungee cord. Scraper was worn & popped under head pulley. The scraper tensioning arm spun around and struck top of right hand.
Tearing down crusher wrapping cords, taking out the plywood for the ramp, deblocking, move conveyor and picking up. Did have safety glasses on.

Cutting off the male end of an extension cord, had extension cord on the table with his hand below the table. The utility knife slipped and caught his left thumb just above the bottom knuckle.
