After turning off a pump at the end of EE's shift at a pond area, the employee was walking to the generator to turn it off and stumbled on unlevel ground twisting right ankle resulting in a fracture.

Employee was walking on level ground when he stepped into a small, 3" depression. The employee stated his knee twisted in an awkward manner resulting in a sprain of the medial collateral ligament.

Employee was struck on the hand/finger by a worker using a hammer to remove core from core barrel.Crew were trying to hammer out a rock in the bit when injured employee re-positioned his grip in mid-swing.

Employee parked water truck next to asphalt refuse pile. Asphalt section rolled down hill while employee was next to driver door area. Material hit employee on right leg. X-ray has revealed a hair-line fracture on bone of right ankle. Employee released for light duty.

Employee picked up a bag containing training materials and felt a twist in her back which didn't present pain at the time of the incident. The next she day wasn't sure if the pain was from cramps or kidney. She did not seek medical attention until June 19, 2011 and she reported the back pain issue to her immediate supervisor on June 20, 2011

During a closing of a septic tank system at a residence in the town site a contractor backhoe moved a dead wood log disturbing a hive of bees that were in the log but that were not evident during the area inspection that morning. The bees came out and attack the employee who was a bystander 60-75 feet from the log. The employee recieved multiple stings on arms and neck.

Employee was moving stabilizer from pipe truck to drill utilizing the winch on the drill. Employee had hold of stabilizer, and the momentum of the tool forced him into the rig, causing him to hit his fingers on the hammer that was stationary in the rod table.

EE was working on the 1800 screening plant when he removed a rock from the tail pulley. The belt rolled back and caught his left hand in the pulley. He then used his right hand to push another employee out of the way, straining his right shoulder. EE started receiving physical therapy on his right shoulder 07/28/2010. Turning this into a medical treatment.

EE had loaded 6 - 500' rolls of HTPE 2" plastic pipe on hydra-crane truck. EE attempting to line up a roll on top of another. Pulled on the 1/2" plastic strap that keeps the HTPE from unraveling. The strap broke & EE fell backwards from top of truck & landed on a wooden pallet that cut the right side of his cheek.

Employee was unloading scrap metal out of a 10 wheel truck. The metal stuck to tailgate as it was being unloaded. Employee lowered the truck's bed and went to loosen the tailgate to release the stuck material. The tailgate released and pinned the employee's left arm between the tailgate and bed of the truck, breaking his left arm.

EE WAS IN THE PROCESS OF REMOVING A RIGHT REAR STRUT FROM A HAUL TRUCK, USING A HYDRAULIC PULLER. EMPLOYEE REACHED INTO THE STRUT AREA TO SEE IF THE PIN HAD MOVED. THE PULLER BROKE AND HIT HIS RIGHT HAND PUSHING IT UP AGAINST THE RIGHT WHEEL DRUM CAUSING A LACERATION TO HAND.
