Employee reported on 10/31/2019 that they had allegedly strained their right shoulder while using a hose at conveyor on 10/30/2019. Went to ER and was released to return to work: 11/1/2019 went to ortho and was given 2 weeks off.
Employee slipped and fell against handrail of crusher and injuring right bicep muscle
Employee was getting out of skid steer, slipped on step and fell fracturing his left forearm. Ground was muddy.
