Employee walked to retrieve a gladhand lock out of the lock bin on the shipping dock. Upon turning around to go load out a truck, the employee felt a pop in his right knee. Pain and swelling ensued soon after. Employee was sent for medical evaluation and assigned a 7-day work restriction.

Taken to Clinic-Xrays & medications. Lift restrictions. Steering wheel flat&no tilt. Tried to simulate. Was suspect & 8/15 WC determined head & neck would be covered not shoulder.

Operator was assisting maintenance on Mill 19 reel chain that had broke. Couldn't get chain on rotary splitter. Operator came up on steps to hold splitter to position to install. Maintenance employee walked down steps to get tools. Operator stated he was rotating rotary splitter to position & it bound up and he had sharp pain in back.

Miner stepping down from step stool missed a step, causing him to fall backwards and strike back of hard hat on floor. Miner was sent to emergency room for evaluation, diagnosed with contusion, minor head injury, and minor neck/back sprain. Was prescribed painkillers and given work restriction of three days.

Temporary employee handling pallets received splinter in right middle finger on Tuesday 03/18. First-aid conducted to remove splinter. On Friday 03/21 employee complained that finger was swollen and painful, employee was sent in for medical evaluation. A piece of the splinter was found remaining in finger, employee was prescribed antibiotic and work restriction.

Employee was loading truck at Silo 33. Employee lowered load out chute into hole. Chute contacted lip of the hole rather than going down into it. Employee reached down with his left hand and pulled on chute to allow it in the hole. The steel rings of the chute pinched the employees left palm resulting in laceration approximately 2 inches long. First Aid was administered.

EE was in the process of cleaning TK96/97 bulk scale with water hose when he stumbled and felt a slight muscle pull in his lower back. After task was completed he parked the water truck and started to exit the truck when he experienced pain in his lower back that restricted further movement. The process of twisting to exit truck caused the resulting sensation of pain.

Employee was climbing ladder at TK13 with a 45 degree angle pipe on shoulder when employee lost contact with 4th or 5th rung causing him to fall to ground level about 4 to 5 feet. Employee received a laceration to right arm at the elbow and minor abrasions to his back. Employee also received a minor cut to crown of head.

He explained that he fell while attaching the sack loops to the forklift. Access to forks on tow motor, and sack spout was being gained by a stack of 3 empty pallets, 4x4 boards, and a piece of plywood on top of the floor scale. As he stepped up onto the pallet configuration the plywood on top shifted causing him to fall.

Employee tripped and fell at dock area. Complained of back pain. Initially thought injury was minor but three days later (7/8/2011) had to go to hospital for back pain. Back pain treated with anti-inflammatory, muscle relaxant, and pain med and employee released. Employee reported back next scheduled shift (7/12/2011) with no restrictions.

Employee started descending from the packaging cart while carrying his large, personal portable radio in his right hand. The employee did not maintain the required 3-point contact as he descended and stumbled, chipping his upper left front tooth. Employee was taken to the dentist. The employee's mouth was x-rayed, there was no damage to the root or nerve and the tooth was filled.

Employee stated that he had become overheated the previous shift (6/23). Despite resting and drinking 4-32 oz bottles of Powerade, he said he was still having symptoms of heat exhaustion and insisted on being seen by a physician. He was transported to the local hospital ER and given fluids and medicine for nausea and headache. He was released at 1 am.

The employee was using a pipe and a hammer to break up clumps of material in an outside hopper. The employee put one end of the pipe on a clump and went to strike the other end of the pipe. The hammer head glanced off the end of the pipe and the employee's hand that was holding the hammer hit the end of the pipe. The employee broke the second digit on his right index finger.

Employee was dumping a hopper into a dumpster. Employee triggered the release with his hand while standing next to the hopper. The hopper tilted forward to dump and bounced back. When the hopper came back, the employee's finger was pinched between the hopper frame and tow motor. The employee's finger-tip was broken and lacerated. The finger required 7 stitches to close the wound.

Scaler exited scaling machine to check face. While doing so, passed by 206 lb rock leaning against face which tipped over onto his leg pinning him to the ground for 45 minutes. Radio was in cab of machine. Driller found him and freed him. Scaler was taken to hospital for assessment of leg. X-rays were taken no bones were broken. Diagnosis was contusion of right lower leg.

EE had parked at fuel tank.While descending from the truck,EE's right foot caught on the last step, causing him to hit the ground hard with his left leg.EE initially thought injury was minor and self-treated with ice and Excedrin.On 11/21/2008, swelling was still present so employee was taken to Doctor and was prescribed an anti-inflammatory medicine.

A mechanic was changing the front tire on a 15 ton Ford dump truck. The mechanic removed the bad tire and leaned the tire against a hand rail. As the mechanic walked away from the tire, it fell and struck the mechanic in the back of the leg. This caused the mechanic to fall backwards, towards the falling tire. The incident resulted in a broken leg.

12/19/07 - Maintenance employee was walking up an inclined travelway when he stepped on a small stone (1") and felt a "Pop" in his knee. Employee did not believe it was anything and did not want a medical evaluation. Later, 1/3/08, the employee reported lingering discomfort in the knee; doctor evaluated and prescribed Celebrex and it became reportable at that time.

Loader operator was mucking out a heading that was shot the previous shift. He scooped up a bucket of material, raised his bucket approx. 3 feet and started backing up when (apparently) an unexploded stick of dynamite detonated. All miners were evacuated from the mine. There were no injuries or property damage. MSHA was immediately notified.

A production employee was going to switch pumps at the sulfuric acid system. He put his chemical gloves on, but did not put his chemical suit and face shield on. When he switched to the backup pump, acid sprayed from a weakened connection on the pump and caused several small burns to his back. He was prescribed an antibiotic to ward off infection.

While cleaning and servicing a Great Western Screen, the employee put his hand down the discharge sock (17") and his finger impacted an airlock. The employee lost the fleshy front tip of his middle finger. The employee was taken to the hospital and treated for the wound. The employee failed to follow the documented company policy for Lock-Out/Tag-Out and was terminated.

He had one hand on handrail and the other hand had tools in it. When he reached the bottom step his ankle gave way. He used the handrail to lower his body to the ground. His supervisor then helped him to the breakroom and called safety to look and report. She asked him did he need medical treatment and he replied no. He was driven home and later seen at the local ER.

An operator cut his right ear on a pneumatic fitting while trying to remove a busted bag from a conveyor. Operator was moving from the backside of the bagger between the bagger spouts to remove the busted bag. Operator did not shut the conveyor off to remove the bag, therefore he was moving through the area quickly in an attempt to remove the busted bag from the conveyor.

The employee climbed backward down (normal routine) off the Grader, placed his right foot first on the ground, turned to place his left foot down and twisted the ankle on the right foot.The Grader was parked on level ground, but the ground surface was somewhat uneven. No lost time from work.

Electrician with 2 yrs mining experience was injured while laying internet cable above ground. Incident was not reported until 3/12/07 when the employee reported the injury and was put on a no work status until 3/28/07. Because employee did not report incident in a timely fashion and physicians were not free in giving medical information, investigation was delayed.
