EE is a dispatcher & works in an office trailer at entrance to quarry. Office was having phone service disruptions & a repair tech arrived. EE went to show tech where disconnect box was. As EE was walking back to trailer, foot caught a ground strap/cable attached to disconnect box & EE tripped & fell. Injuries were believed to be minor. On 5/23/23 EE was still having pain & had MRI

Showing all 21EE was cutting a piece of belt for the jaw feeder. EE was holding the rubber with left hand and cutting with the right. While cutting, the utility knife slipped and struck the EE on the left inside forearm. EE was cutting towards self. EE sustained a laceration that required 5 stitches. EE continues to work and did not miss any days of work.
EE was trying to remove a large piece of metal from the conveyor magnet. Typical pieces were not this large. The EE used a crowbar to open a gap between the metal and the magnet. When trying to pry the piece off with a shovel, the shovel slipped causing the metal plate to snap back to the magnet, crushing the EE's left thumb.
EE was using a sledgehammer to clear a chute. When they struck the metal with the sledgehammer, their little finger was caught between the handle and the metal, causing a fracture.
EE was removing the bucket from the skid steer. EE had the bucket lifted off the ground. When ee got it unhooked the bucket came off and landed on feet.
Annual hearing test results indicated an average change of hearing in the left ear of 12.6 dB and a change of 14.3 dB in the right ear.

EE was making repairs to a D8 Dozer. As he was dismounting the dozer, he slipped on the dozer step and cut his right shin.
Employee was using a utility knife to cut a piece of urethane rubber. The utility knife slipped on the rubber causing the employee to cut his left forearm.
Employee was testing wiring in the electrical panel of generator 7633, an arc flash occurred sending a "ball of fire" out of the panel. The fire caused burns to the top of both the employee's hands.
Employee was on the mine property after hours cutting wood. He collapsed and died on the mine property. Coroner ruled that he died of natural causes. Coroner determined that he died from massive heart failure from enlarged heart and advanced emphysema.
EE was standing on the belt guard trying to loosen a mantle nut on the main frame. He had both feet on belt guard. He went to step off onto the lower platform and slipped. He tried to catch himself on the guard rail but missed striking his upper right side rib cage on the adjusting bolt on the main frame of unit.
While trying to loosen a 1 1/2 in bolt on a jaw crusher with a wrench, the employee felt a pull and pain in his upper right bicep.
While torching metal an amber flicked into his boot causing a 3rd degree burn to his left foot.
While exiting loader, employee's foot slipped. While grabbing the loader to stop his fall he felt a pop/twist in his right shoulder. EE did not report incident until 07/24/07. Employee was evaluated at occupational clinic & given restriction of Limit above shoulder work. No restrctions effective 07/27/07.
While operating loader, the BAT warning light came on & alarm went off. EE looked behind him & saw smoke. EE called supervisor. They attempted to put out the fire with a fire extinguisher but were unsuccessful. Fire Dept responded. Loader was a total loss.
Loader step was covered in snow. Employee slipped while exiting the loader. Diagnosis is first degree right ankle sprain. Able to return to work immediately. Told to take Motrin and wear boots that supported the ankle.
Utility knife slipped while cutting rubber skirting. Utility knife cut miner's left ankle received 3 stitches.
EE was climbing out of the loader and step on a stone that was on the step and slipped and fell. EE suffered a contusion to the left elbow. He was evaluated at Memorial Center for Occupational Health. Dr.indicated restrictions of maximum lifting, pushing and pulling of 5 lbs for the left arm 20 lbs total with limited use of the left arm.
EE WAS CUTTING A HOLE IN A STEEL PLATE. IT WAS WINDY & SPARKS FROM THE TORCH BLEW BACK & BURNT THROUGH HIS CLOTHES & BURNED HIS BELLY. WENT TO DOCTOR ON 5-1-02. HAD 2ND DEGREE BURN & PLACED ON RESTRICTED DUTY.
TRYING TO UNHOOK CHAIN. EE NEEDED SOME SLACK. FINGER GOT PINCHED BETWEEN FORK & CHUTE.SENT IMMEDIATELY TO HAND SURGEON FOR DEBRIDGEMENT & TENDON REPAIR. TWO STITCHES REQUIRED.
