Employee was removing fluid coupling from 10 Aux drive. Fluid coupling slipped out from between the couplings and fell on employees hand. Employee received stiches and broken finger.

While getting out of the pickup in 158 lot EE employee stepped down on a rock causing the employee to twist EE's ankle. Became reportable on 05/15/2026 when diagnosed with torn ligament.

While enlarging a hole for an E-Stop using a step bit, the bit snagged an aluminum label plate. The plate spun unexpectedly, resulting in cuts to the operator's thumb and index finger.

Employee was pulling on a rock with the scaling bar when they felt a pain in their stomach. They went to lean on the cage and felt a sharp pain in stomach. This became reportable on 03/13/2026.

Employee was cleaning out the bottom level of the feedend and removing unused metal from feedend area. As EE was dragging the piece of metal out, EE tripped over another piece of guarding EE had previously moved into a pile to dispose of. EE fell onto a piece of angle iron on the pile and cut the back of EE's knee.

An employee slipped and fell while descending the wooden stairs near the office trailers, landing on their arm and injuring their shoulder. During the fall, their foot slipped forward, causing them to land backward on their arm near the elbow, resulting in pain extending to the shoulder. A medical evaluation confirmed a small fracture in the left shoulder.

Employee was performing cleaning and maintenance. They began work around 7 pm and was discovered by coworkers on the ground below the area of work. It is believed they fell.

Employee was dumping a load at the tailings dump in the Mississippi Mine. After dumping the load EE watched for the light on the dash to go off signaling the bed was down. EE began to exit the mine and when EE got to the blacksmiths shop EE felt the bed contact a brow and ripped the bed off

Employee was using vac truck to empty rail car. Had to use an air lance to break up the material. When the lance was moved to a new location it shot material back in to the face.

Employee was raking oversize rock from the grating at the discharge of the Dryer. As EE was prying rocks from the grating, a rock fell from the end of the dryer kiln, bounced off the grating, striking EE in the mouth. This resulted in the chipping of two teeth and loosening of four teeth. Employee will need dental work to repair teeth.

Employee was cleaning out the vault area of the kiln, scooped up a large piece of Hot Coating. It rolled over the back of the bucket striking a hydraulic fitting, causing it to break and spray Hot hydraulic oil , causing a flash fire that was extinguished in less a minute.

Driver was offloading at the Vertical when the line became plugged. EE shut down the air and went to work the plug. The cam lock came loose, and lime blew into EE's face. Driver was wearing goggles. Employee was prescribed medication for EE's eye.

Employee was working on MRH #2 separator, employee was crouching under neath the catwalk when they felt the lower back tighten up. Employee reported incident on 04/21/2025, then received restrictions on 04/29/2025.

Employee was descending the MROT storeroom stairs carrying boxes. Once EE got to the bottom of the staircase, started walking to leave storeroom, and tripped over pallet that was in walkway causing EE to fall on EE's arms/shoulder. Employee had an MRI that showed a broken bone in the shoulder as well as a torn labrum after follow-up with orthopedic on 4/17/2025.

Haul truck operator was getting loaded and on the last bucket EE experienced a jolting feeling that caused pain in EE's back.

Employee was cleaning along 19 belt when a gust of wind blew stone dust into EE's eyes. EE washed out eyes and assumed there would be no further issues. The eye irritation continued and got worse over the following days. Employee was given prescription eye medication on 3/8/2025.

Employee was climbing ladder to go to upper level and when EE got near the top, employee stated wind blew lime dust in EE's eye. Employee was wearing safety glasses.

Scale Attendant fell on ice going to car. Employee has been performing normal job duties but has had consistent discomfort. Employee became reportable on 3/14/2025 after a follow up doctor's visit indicated a nondisplaced fracture of the left tibial spine requiring restricted duty.

Employee stepped on a piece of ash ring debris and sprained right ankle. Orthopedic doctor diagnosed torn ligaments, prescribed boot, crutches and physical therapy and restricted duty to no climbing steps or ladders. MLC is accommodating restrictions.

Employee was running the grader on 114 and had the blade in the float position. Turned off 114 into a heading and pulled the blade up. The blade caught a toe and it jolted the piece of equipment. Employee noticed discomfort in their back.

While working on a cylinder on a diverter gate, employee leaned over to see if the shaft was going to clear when cylinder/arm broke striking employee in left jaw.

PROT Employee was cleaning RK2 Baghouse on 3-11 shift. At one point EE bent over near a cell discharge damper to pick up trash. As EE went to stand up with EE's hard hat on, EE struck EE's head just under the edge of the hard hat at a force hard enough to cut the back of EE's head requiring 3 staples.

Employee was doing the end-of-shift report and was checking if they were currently crushing 2". As EE looked up from the platform to the underside of the bins to see if the 2" slide gate was open, EE felt material fall into their eye. EE washed out the eyes and felt very little irritation, but the feeling of something still in the eye. Employee went to the eye doctor on 11/15/2024.

Employee was running 990 loader and noticed soreness in knee midway through the shift. Continued to work and noticed pain getting worse as shift came to an end. Employee has history of knee pain so unsure if injury was work-related at this time.

Employee was performing the tasks of lime loader helper, moving and loading railcars. Employee believes that EE hit EE's knee after tripping and landing on the knee while entering/exiting the top of a railcar. The employee stated that EE didn't report the incident initially, believing that there was no injury of concern. However, it appears the injury may have become infected.

Employee was walking to exit the preheater. As EE was going to exit and started to adjust to get out, EE's foot slipped off the top of the horizontal dome cooling pipe EE was standing on and started to fall. Employee stuck out EE's arms to try to stop falling. Employee noticed afterwards that EE's right shoulder was hurting. MLC was notified 10/28/2024 of medical care.

Employee was scaling inside the preheater on the wall of preheater. EE went to scale some material off the wall, and then felt the right knee pop. EE said felt a discomfort when EE started to walk, and a sharp pain was apparent when EE would take a step. This was felt mostly when EE extended leg walking.

While scaling a rock off of a ledge on the pillar the rock twisted and contacted employee's scaling bar causing the bar to contact EE's right arm and overextended EE's elbow. There was a discrepancy in thought as to the extent of the injury, however, it does appear there may have been a tear.

The employee tapped a pipe on the floor and when EE did, material that was in the pipe flew up into EE's eyes. Operator had the PAPR on but had the shield up at the time since EE hadn't started bagging. Employee was taken to eye doctor and given a prescription.

Employee stood on chair to reach smoke detector, overextended and fell hitting head against door hinge. Employee cut ear requiring stitches and was returned to full duty.

Employee was stepping off ladder, after cleaning HRH loadout camera, EE felt a pain in EE's left knee. This pain got worse, which EE thought EE hyperextended EE's leg. EE then reported it to EE's supervisor. Employee continued to work until November 24, 2023, when EE had surgery to repair a tear.

Employee was struck in the mouth by a 6' scaling bar. The employee was attempting to pull rock with the bar when it slipped and struck the employee in the mouth. The employee suffered a laceration to the lower lip resulting in three (3) stitches. Employee returned to work.

Employee was working to remove a separator gearbox and felt a muscle pull. Employee was diagnosed with a hernia, however, initial restrictions did not interfere with work duties. Subsequent restrictions, however, issued on 9/12 interfered with work duties making this reportable. Employee will also receive surgery.
