Employee was repairing a power cable (S/O 480v) that was pulled out of strain relief. He was moving the cable to get slack for the repair. There was cut or worn spot that shorted out causing an arc that caused first and second degree burns on right hand and thumb.
While pushing a roller back into its bracket with force, roller in hand slipped hitting a piece of metal.

While using a bucket from a 980F Wheel Loader, this employee and another got into the bucket was hoisted up to reach aprox. 7 feet to change a whiper on a conveyer belt. Driver of the 980F had a foot that slipped off the brake, and the machine went forward pinning the injured employee.
While slimbing down a ladder a steel pole fell off the first deck and hit employee in the head.
LOADER STEPS WERE WET, WHEN CLIMBING OFF EQUIPMENT FROM 3RD STEP FROM THE BOTTOM INJURING LEFT THUMB AND LEFT KNEE. DOCTORS DIAGNOSIS WAS SPRAIN TO BOTH BODY PARTS.
WHILE PULLING PAD HE REACHED UP TO PULL A CHAIN THROUGH THE LOOP ON THE LOADER BUCKET, STRUCK RIGHT ELBOW ON BOLT ON THE TAIL PULLEY ADJUSTMENT BOLT.
EE WAS PUTTING A SET OF LEGS UNDER A CONVEYOR, HE THEN SET THE WEIGHT OF THE CONVEYOR ON THE LEGS. WHILE STANDING ON CATWALK TO SECURE LEGS, THE LEGS SLIPPED, CAUSING HIM TO HIT HEAD AND KNOCK ING HIM DOWN.
EE WAS LOADING TRUCKS WITH GRAVEL ON ROUGH TERRAIN, HE WAS BOUNCING AROUND IN THE CAB OF THE LOADER, CAUSING HIM TO HURT HIS BACK.
