REMOVING GEAR BOX FROM CONVEYOR SHAFT WITH A HAMMER AND CHISEL WHEN ROCK FLEW BACK AND HIT HIM IN FOREHEAD. HE RECEIVED A CUT THAT REQUIRED 4 STITCHES.
ON WED 09/15 EMPLOYEE WAS HELPING TO WELD. HE REPORTED TO WORK ON THURSDAY & SAID HE HAD HURT HIS EYES YESTERDAY & WORKED FOR APPROX ONE HOUR & WENT HOME. HE WAS TOLD MORE THAN ONCE BY HIS SUPERVISOR TO GO TO THE DOCTOR BUT SAID IT WOULD BE OK. HE DID NOT SHOW FOR WORK ON FRIDAY & WORKED A FULL DAY OF MONDAY BUT FINALLY WENT TO DR ON TUESDAY TO FIND THAT HE HAD A WELDING FLASH.
EE WAS OPERATING AN IMPACT DRIVE WHEN IT SLIPPED OFF BOLT & CONTACTED THE PALM OF THE LEFT HAND. EE RECEIVED 5 STITCHES.
EE WAS CLIMBING LADDER TO HIS WORK AREA AND STRUCK HIS ARM ON A PIECE OF STEEL CUTTING IT.
THE EDGE OF THE BELT WAS CAUGHT. WHILE USING A PRY BAR TO DISLODGE THE BELT, THE PRY BAR CAME DOWN AND STRUCK EE IN THE NOSE.
EE WAS INSTALLING BELT AND WHEN EXITING CONVEYOR HE SNAGGED THE EDGE OF THE STEEL FRAME CUTTING HIS HAND.
