During maintenance, a miner was struck by the boom of an excavator that was being used to position crusher parts; causing him to lose balance and fall approximately 11 feet. The fall resulted in several broken bones. MSHA arrived on site the afternoon of the accident and started an investigation. *Employee died on 5/26/2015.
The injured employee was climbing the stairs of his equipment when his foot slipped off of the step. He banged his knee on the equipment as a result of his foot slipping off the step.
The injured employee slipped off of the step of the loader he was operating. He caught himself using the handrails, but strained his back in the process. He was operating under normal conditions.
The employee was performing some repairs on the Kue Ken jaw crusher at the facility. While exiting the position from which he was working, the employee twisted in such a way that he aggravated muscles in his lower back. **The employee did not report this injury to a supervisor until 8-14-07**
