INSTALLING PULLEY AND FINGER GOT CAUGHT BETWEEN PULLEY AND CONVEYOR FRAME. BROKEN FINGER.

emp. felt a sharp pain in right shoulder while trying to lift the cab protector gate on a loader. emp has missed work, pending further medical eval.

Employee hurt his knee while he was walking through mud, when he slipped and fell.

Emp was using a utility knife, it slipped and cut his left arm. Stitches required.

emp got a piece of metal in his eye, but not sure when it happened. emp feels it must have been at work, but can't give a specific incident.

Emp. was pulling a cable and had his knee in place for support, when he began pulling, he felt a pain in his knee. Emp did not seek medical treat at the time of the accident, but later went to er for swelling and soreness. Emp missed 1 day of work, but is still on light duty.

Emp was grinding metal on 3/12. emp reported on 3/13, he thought he had pink eye. he went to ER on 3/14 and they found metal in eye. he was refered to an eye doctor on 3/16 who kept him off work until 3/19 when he returned without restrictions.

EMP STRAINED HIS SHOULDER WHILE TRYING TO REMOVE A FALLEN TREE OFF OF THE HAUL ROAD.

emp was using a port-a-power when the wood blocks kicked out and struck him above and below left knee. emp has not missed worked but is on restrictive duty.

Removing a piece of drill pipe that needed replaced and as employee was lifting the pipe he got a sharp pain in his left arm-bicep area.

Employee hurt his right ankle and shoulder while trying to climb into his loader and slipped.

Employee was digging out a stone chute and felt sharp pain in his left shoulder, arm and hand.

Emp. reported he was working on equipment and felt a pain in his arm, like a bug bite. emp did not go to doctor until 7/31 and was given a note with no restrictions or medical treatment. on 8/12/2008, emp still felt pain and returned to the doctor, and on 8/13 emp had a non metal object removed from his arm. medical treatment and four stiches.

EMP WAS USING A UTILITY KNIFE, CUTTING TOWARDS HIMSELF AND CUT HIS KNEE. EMP MISSED 1 DAY OF WORK AND HAD ONE SUTURE.

EMP WAS PICKING UP STONE, FELL BACKWARDS, TRIED TO CATCH HIMSELF AND FELT PAIN IN SHOULDER.

EMP WAS CHANGING SCREENS AND BUMPED HIS RIGHT KNEE. WHICH BECAME SORE. EMP MISSED 2 DAYS OF WORK, 5 DAYS OF RESTRICTED DUTY AND RETURNED TO REGULAR DUTY ON 4/28/08.

EMP FELL OFF A STEP LADDER WHILE PERFORMING MAINTENACE/REPAIR ON A STEAM JENNY. STEP LADDER WAS NOT FULLY OPENED, IT WAS PROPPED AGAINST EQUIPMENT.

EMP WAS DESCENDING A 992C CAT LOADER, SLIPPED AND FELL, HURTING LEFT ELBOW AND BUTTOCKS.

Emp was lifting a block and hurt his back. Emp is still working but has restrictions.

Employee was getting cardboard blast hole casing from a shelf. He was turning to toss the casings to the floor and lost his balance. He fell approximately 3 feet to the floor.

EMP. SLIPPED WHILE GETTING IN TO TRUCK AND SPRAINED HIS SHOULDER. EMP. DID NOT SEEK MED. TREATMENT UNTIL 12/27/2007 WHERE HE WAS GIVEN A PRESCRIPTION AND RESTRICTION ON LEFT ARM.

emp stated his wrist was sore from beating ice off of steel. emp did not seek medical treatment until 12/26/07 where he was given a presciption and limited use of his wrist.

Emp was walking from truck to excavator, slipped on snow/ice and jammed right arm. Emp was given percocet and had to wear a sling for one work day.

Emp had a cutting edge of a Cat 980 bucket fall on his foot. Report was not submitted until 10/31/07. Emp missed 1 day of work and was then on restrictions until 11/12/07 when he was released. Info was attempted to be entered via the Internet, but MSHA's online filing was not operational.

Burn on left leg due to cleaning boots with steam jenny, EMP DIDN'T REPORT THIS UNTILL 10/26/2007.

EE tripped on rock and tore ligament.

EE was shoveling tunnel belt and felt pain in his back. EE took off work fri 8/31/07 without doctor note. EE was told he needed to see a panel doctor, who then put him on restrictions.

Emp hurt left shoulder while climbing ladder.

Employee was changing tires and swinging a sledgehammer and has a bulging disc. Employee did not report this injury until 8/17/2007.

Emp was stung by a bee. Stated he did not feel well and went home. He called his doctor who told him to take over the counter medicine. No doctor visit made. Emp returned to work on 7/10/07 at normal time. Emp quit the job at 8 am on 7/10/07. Emp never showed anyone where he was stung.

EMP WAS GETTING OFF LOADER, STEPPED ON A BLOCK OF WOOD, LOST BALANCE, AND STRAINED HIMSELF TRYING TO REGAIN BALANCE.

Employee was lifting parts and felt pain in back. He did not go to chiropractor until 6/29/07, where he was informed he would need more than one visit.

EMP WAS LIFTING 40-50 LBS ANVIL AND FELT PAIN IN HIS BACK. EMP MISSED WORK FROM 6/13 TO 6/15. EMP RETURNED TO WORK WITHOUT A RELEASE ON 6/18 AND WAS SENT HOME TO GET ONE. EMP RETURNED TO WORK ON 6/20 WITH RESTRICTIONS.

EE was welding and hot slag got in under his helmet and into his right eye

Emp stated he got out of bin truck and felt a pop in his leg. He did not report accident at that time, but did not show up for work on 1/29/07. He did not report injury to me until 2/2/2007. EMP ALSO STATED DID NOT FEEL PAIN UNTIL SUNDAY OR MONDAY.

EMP WAS STRUCK BY A ROLLER BEING MOVED BY CRANE. EMP REPORTED INCIDENT ON 1/26 BUT DID NOT SEEK MEDICAL TREATMENT UNTIL 1/29. HE WAS GIVEN A NOTE TO BE OFF WORK TILL FEB 1, FOR A SOFT TISSUE INJURY

FOREIGN BODY IN EYE WHEN STONE HIT OUTSIDE CORNER OF GUARD AND HIT EMP IN EYE

Employee was putting away a chain wrench and got his finger pinched between the chain and wrench.

EMPLOYEE SLIPPED ON WET SAND THAT WAS ON CONCRETE, FELL TO THE GROUND (SAME LEVEL) AND BROKE HIS HIP.

EMPLOYEE WAS CUTTING BRUSH AND TREES, AND STEPPED FORWARD INTO THE BLADE OF A CHAIN SAW. REQUIRED STITCHES.

Employee was dragging a 2" air line and stepped into a gulley and fell. He twisted his knee while falling to the ground and his head struck the ground. His hard hat flew off and his head hit the ground again, without his hard hat.

EMP WAS CHECKING HP ON A MOTOR, SLIPPED AND CRACKED KNEE CAP

EMPLOYEE WAS OPERATING EUCLID 0029 WHEN DRIVE SHAFT CAME APART. WHEN THE DRIVE SHAFT CONTINUED TO SPIN, IT KNOCKED OUT BOTH REAR BREAKING CYLINDERS. THE TRUCK DESCENDED A GRADE, AND PARTIALLY UP ANOTHER GRADE, THEN ROLLED BACK INTO THE HIGHWALL.

EMP WAS WORKING ON PLANT AND TWISTED ANKLE.

Emp was making repairs at plant, when hammer slipped out of his hand and struck his left knee.

Employee was walking on the catwalk of his euclid and slipped on oil. Employee caught himself and didn't fall, but hurt shoulder. After treating with doctor it was found he tore his rotator cuff.

EE SMASHED HER FINGERS WHILE TRYING TO CLEAN OFF TAILGATE OF HER DUMP TRUCK. (WAS NOT USING A TOOL, JUST HER HANDS. CUT RIGHT RING FINGER - STITCHES REQUIRED.)

Employee was struck by moving vehicle. (EE was standing on soft material when pickup truck ran over him. EE sank down into soft material, so no fractures - bruises to legs only.)
