Showing all 16Employee was turning the backhoe off to go to lunch. Foot slipped off the track and scraped back of leg.
Using 3 come-a-longs to lift the plant jack assembly, the come-a-long came loose causing the part to swing back striking the employee, causing employee to fly back approximately 4 feet.
The injured was mashed by loader bucket against conveyor belt.

Euclid lost it's brakes as it was going up the ramp and started rolling backwards. It hit th ebig rocks being used as a berm. A tire blew out as Euclid hit another rock and came to a stop. Driver was still in the vehicle, turned the motor off and put it in gear. Got out, walked up to the top of the quarry, then was taken to dr. Only bruises shoulder/leg. Was wearning seatbelt.
Injured was helping another ee fix washer and hurt himself, but about 6 months ago, but did not report to anyone. On 9/5/07 injured was helping to put bearings on washer and aggravated.
Injured was struck by a steel plate opening and shutting a flood gate when bagging material . Material was stuck he opened gate and fell on hand 4 x 6" plate.
Subject was helping to tarp a load on a trailer, backed up, slipped and fell off trailer landing on right shoulder.
EE WAS USING A BAR AS A LEVER. THE BAR SLIPPED, HITTING LITTLE FINGER OF RIGHT HAND ON STATIONARY OBJECT.
EE WAS STACKING 100 LB BAGS ONTO PALLET, FELT A PULL AND BURNING UNDER RIGHT ARM. WENT HOME THAT DAY CAME BACK TO WORK ON 11-5-01 WORKED ALL DAY ON TUESDAY 11-6-01 ABOUT 11 AM CAME INTO OFFICE TO REPORT THAT HE WAS HURT AND WANTED TO GO TO DR.
EE WAS STACKING BAGS OFF THE CONVEYOR BELT, GOT LEFT HAND CAUGHT BY BELT.
