After conveyor did not start, Employee went to check the reset button on the electrical panel. When they bent down to see it, their hand was close enough to the panel that they believes sweat dripped from their hand and electricity jumped from panel to their left pinky finger.

764 Power screen overheated. EE opened radiator cap to check fluid level. Under pressure, fluid sprayed out and got onto EE's arm and face.
Carrying 4x4 oak blocks (one in each hand) to chock wheels of conveyor for maintenance. When setting down block in right hand, block fell from hand and twisted EE's right forearm. Diagnosis was strain of right bicep.

EE did a lot of pulling, pushing & lifting during the week, helping to set up crushing spread at K-pit. On Friday the 18th, he had pain on left side of groin. He reported HR on Monday & had hernia diagnosed on Sept. 18. No treatment had been given, no lost time, no restrictions until surgery on November 7, 2008.
EE has been a laborer & equipment operator. He reported on 3-29-07 that over the last couple of months, his pinkie & ring finger are numb. He has been diagnosed with carpel tunnel in the elbow. Surgery is scheduled for 4-17-07. He has no lost time up to that date. This report prepared when notification of surgery was confirmed.
Employee & co-worker were prying a piece of plate steel up into place under the cone, above the belt. The crowbar slipped and hit employee above the left eye causing the cut.
EE is a long time loader operator. he reported on 6-14-04 that over the last nine month his left hand has become numb and tingling. he has been since diagnosed with carpal tunnel of left wrist. surgery is scheduled for 8-16-04. he has had no lost time up to that date. this report prepared when notification of surgery was confirmed.
EE WAS HANGING EMPTY BAGS ON FILLING SPOUT OF BAGGING MACHINE. THERE WERE NO APPARENT REASONS FOR INJURY OTHER THAN HE WAS MOVING HIS HAND AND ARM TO LIFT ONE EMPTY BAG AT A TIME. EE FELT PA IN AND HAD SUDDEN SWELLING ON TOP OF HIS HAND.
EE WAS USING AN ELECTRIC GRINDER TO REMOVE PAINT FROM A PIECE OF METAL THAT NEEDED WELDING. EVEN THOUGH HE WAS WEARING SAFETY GLASSES, A SMALL PIECE OF METAL MUST HAVE ENTERED HIS EYE.
EE WAS HELPING REPLACE A SCALPING SCREEN. HE WAS HOLDING IT UP FOR POSITIONING USING A 5'PRYBAR,THE SCREEN MOVED, PINCHING HIS FINGER.

EE WAS FIXING CRACKED HYDRAULIC FITTING SLIPPED ON OILY SURFACE FELL SIDEWAYS INTO BRACE, HIT RIBS. HE THOUGHT THEY WERE BRUISED, 5 DAYS LATER HE HEARD THE RIB CRACK & FELT PAIN
