Showing all 14Removing a seized bolt from the Terex screen plant. Grinder was the best option to remove. While cutting the bolt, EE was grinding when sparks attached to EE's clothing under EE's safety vest and caught EE's t-shirt, flannel shirt and safety vest on fire.
While backing up with head turned back, the dozer shifted on a rock suddenly and hit another rock which caused a whipping action & strained EE's neck.
The ground had some elevated areas where a person could stumble and fall if not paying attention to the surroundings. Employee sustained a back strain from tripping and falling next to the #2 screen plant.
Employee adjusting and pulling a sample. The sampler slid off the blocks and landed on employee's right thumb. Cut right thumb, received four stitches.
Lifting guard into place. Tweeked back. Ice the area and restrict movement and lifting.
Conveyor belt stopped due to material build up. Employee went to clear material and belt rolled backwards. This caught employee's hand and pulled the arm into tail pulley. This broke employee's forearm. *After investigation it was determined the belt tightened back onto the roller after material was removed which trapped EE's forearm.
Releasing pin that holds oxygen tank retainer pin came loose striking employee in mouth
Releasing cam locks on hose connected to water pump. Steam released the hose burning left side of body.
Leaned over to close cab door on loader, pulled muscles around rib area.
Employee was welding on a roller on the feeder conveyor when he twisted his knee.
Setting chute to screen plant. Chute was checked to insure it was secure. Crane was unhooked from chute. Hook caught chute as it was moving out of the way and dislodged the chute. Employee and chute fell to the ground appx. 3-4 feet. then fell over. Employees ribs were bruised/broke and two fingers dislocated.
INDIVIDUAL WAS OPERATING A PADDLE WHEEL SCRAPER REMOVING OVERBURDEN FOR A GRAVEL SOURCE. THE TERRAIN WAS ROUGH CAUSING IRRITATION TO THE NECK AND BACK.
WHILE FEEDING CRUSHER WITH DOZER PRE EXISTING BACK CONDITION WAS AGGRAVATED BY THE CONSTANT JARRING OF RUNNING DOZER
