Employee as working on replacing the #1 drive motor for the screen. While torquing the bolt, the motor pivoted, causing EE's left hand to be caught between motor mount and pivot point; resulting in a fracture to EE's left thumb. EE went to clinic and EE's thumb was splinted.

Showing all 41Employee was removing debris from cutter in cutless area. When removing a large rock the rock shifted, smashing the employee's left middle finger. Employee was wearing gloves. Went to clinic for evaluation and received five stitches. Employee was also placed on restricted duty.
Employee was cutting a piece of 2" angle iron with a Milwaukee grinder. A piece of the grinder broke off and kicked back into the employee's right leg cutting EE's knee. Grinder did have the guard on at the time of the incident. Employee went to clinic and received four stitches.
Employee was operating mobile equipment (CAT 980K loader & John Deere 460 Haul truck) and came to work the next day complaining of tenderness in EE's back. Employee went to clinic and was placed on light duty on 6/16/25. Employee was re-evaluated on 06/27/25 and released to full duty.
Became reportable on 7/28/25 when employee had hernia surgery. Employee was attempting to put return roller back into bracket by raising the roller with their legs. The roller then came out of the bracket on the right side and fell to the ground. The employee jerked their legs when the roller came out and they felt instant pain in the left groin area.
Employee was removing battery cable to lock out the John Deere 318G Skid Steer when the wrench EE was holding made contact with the the positive post and EE's wedding ring contacted the frame causing a burn to EE's left ring finger.
Employee was pulling a 2" hose, 50' long to wash out sand from under the tail pulley on #14 belt. EE's back began to hurt after. On 12/06/24 it still hurt, and EE reported EE wanted to go to the doctor where EE was diagnosed with a lower back strain & placed on restricted duty.
Employee was trying to tighten the tension adjuster for the tail pulley on the feeder. EE was using a 3ft pipe wrench to tighten the nut and applying pressure down when the wrench slipped causing EE to strike chin on the tail pulley guard. Employee received a laceration to their chin that required three stitches.
Employee was removing & rolling up approximately 800 feet of 1 inch cable from starboard anchor winch when EE felt a pop in their right forearm; causing immediate pain to EE's forearm, bicep & shoulder. Employee was taken to clinic and placed on restricted duty.
Customer truck driver smashed their left thumb in the tailgate of their truck when cleaning off excess 57 gravel.
Employee was operating the manlift when something blew into EE's left eye. Employee had been working on the gravel system installing a piece of skirting prior to the incident. Employee stated EE was wearing safety glasses. Employee went to clinic on 7/31/2023 where EE was diagnosed with an abrasion in EE's left eye and issued prescription eye drops.
Employee was getting a sample of sand from the asphalt sand pile when the wind blew a piece of sand into left eye. Employee rubbed the eye then went and flushed the eye. Employee went to clinic where they were diagnosed with a scratched cornea and issued prescription eye drops.
Employee went to step out of the All-Flux System when EE's left foot caught a probe that was sticking out inside the system. The probe went through EE's boot and into the top of EE's foot. Employee required one stitch and was placed on light duty.
Employee was outside the fab shop driving the sky-trak, moving a container when something went into left eye. Employee wiped eye with hand & flushed eye with eye wash but still felt something in eye. Went to clinic & a small piece of debris was removed and employee was issued a prescription antibiotic.
The employee was performing maintenance on a sand dryer. During maintenance the employee was cutting zip ties off of electrical wires with a collapsible knife. While doing so the employee cut the top of right hand resulting in five stitches.
Employee was replacing screens on the shaker deck above the load out bins on the rock plant. While installing the new screen, the injured employee and another employee were lifting the screen into place when the injured employee felt a pop in the right upper bicep.
Employee was leaning against a ladder with left foot on the bottom rung. The ladder was leaning up against a job box on wheels. The job box shifted causing the employee to lose balance and strain the right knee. Employee went to the occupational clinic where EE was placed on light duty.
Employee was grinding on a piece of metal when a piece of metal went under EE's safety glasses and into EE's left eye. The employee went to the doctor where they removed the piece of metal on the surface of EE's eye and issued prescription eye drops.
Employee was checking the pump gauges on the pump dock next to the delignite system when employee twisted left knee. The injury became reportable on 08/24/2020 when the employee was placed on modified duty.
Employee was reaching into a job tool storage box for a tool and while pulling arm out EE caught right elbow on a sharp corner of an aluminum welding rod box cutting EE's elbow.
Scrap metal was being drug off of an old barge using chains hooked to a loader. As the metal was being drug off the barge it caught a lip on the barge and caused the piece of metal to pivot upwards and rotate. As it rotated it fell on top of an employee's foot breaking 3 bones. The employee is on light duty at work.
Employee was assisting with the removal of conveyor leg, the bottom portion had been cut through with a torch. They started to remove the upper section and the leg sprung forward, striking the employee on the hand.
A Miner was attempting to uncoil cable from a suspended spool, when it became disconnected from its rigging, striking the Miner. The Miner sustained neck and leg injuries
Using a 1/2" drill to tighten a splice. The drill spun my right hand around when the nut bottomed out.
An employee tossed a bolt to a coworker, the bolt struck the coworker in the head causing two lacerations that required stitches.
