At approximately 7:20PM the shift supervisor was conducting work area inspections, finding a fall of ground in the 3800 ramp. The fall of ground was above the anchorage zone in an active working area that impeded passage.
After tightening a casing connection, the employee put EE's hands in the derrick before the pipe settled, pinching EE's thumb between the string and the table. Resulting in a half inch laceration.
During a tail screen change at the West Dedicated Plant, an employee was potentially exposed to HCN. After the task, the employee reported lightheadedness and nausea, was evaluated at the onsite clinic, and transported to a hospital. No injury or illness was diagnosed.

Employee was attempting to adjust bolt alignment fingers on bolter using a hammer, while doing so the employees left hand pinky finger got caught between the hammer and the alignment finger causing a laceration. Employee received 7 stitches.
An employee was working on an Auto Valve, when the handle was pulled, it came loose and the employee lost their balance causing them to fall. Resulting in a fracture on fibular head.

Employee had a pipe wrench up to chest while working on a pipe and was trying to use body weight for leverage when employee felt a pop in ribs. On 1/13/25, the employee was taken off of work after further medical evaluation.
A haul truck operator was jarred, while being loaded by a loader, when the loader bucket made contact with the bed of the haul truck. On 1/16/2025, the employee was taken off of work due to continuous pain.

While an employee's (EE) left arm was supporting a drifter on its side during re-installation on the Bolter Boom, additional mechanics began to push the drifter into place with the EE's arm still holding the drifter. This caused the EE's arm to be pinched in between the drifter and the slide, resulting in swelling and soreness to the EE's arm.
The employee (EE) was using a paint stick to begin marking boot holes following a heading inspection. The EE's right foot slipped on uneven ground causing their weight to shift left. The left foot also slipped, and the EE began to fall to the ground. A rock came between the ground and leg just above the left calf area. The impact resulted in a fracture to the lower left fibula.

An Arc Flash occurred as an employee (EE) attempted to throw a transfer switch that was under load. The flash blew the enclosure door open. This struck the EE in the face and forced the EE backward. EE received minor burns to the left cheek and neck while sustaining facial injuries to the mouth and teeth.

Employee was entering mine site for start of shift and felt something hit their eye. Prescription medication was later given to prevent any infection due to evidence of a foreign object that had been in the eye.
Employee was exposed to nitrogen dioxide on 5/15/24. On 6/6/24, it was determined that the employee was experiencing respiratory related symptoms due to this incident and additional medical treatment was needed.

EE was finishing blowing off load of Cyanide into ADR tank. Went to back of truck and Cyanide Crystals fell from top of Storage tank and hit EE, the trailer, and the ground. The wind was blowing toward back of tanker. EE was not sure if EE had inhaled any or not but had a co-worker watch EE disconnect then go get the Supervisor. EE then called mayday.
