The accident is questionable. Employee strained his back on Jan 10, 2011 while chipping ice out of his loader bucket. He reported the accident on 1-20-2011. He worked from 1-10-2-11 to 1-14-2011 without any lost time. He has been laid off since 1-14-2011 and he finally reported his accident on 1-20-2011.

Injury was watering location around Crusher he was told to stay away from soft spot until cobble rock was placed in hole and covered. He forgot and backed next to hole where it was soft & the right side of water truck sank. The load shifted and truck tipped over on right side. He climbed out and seem to be ok. Worked 7/10/09 lite duty. went to dr 7/12/09/
