Showing all 14Lifting a chained ecology block that slipped and temporarily pinned employee's left leg.
Miners were dismantling plant to move to new location. When lowering conveyor to ground, it is surmised that the conveyor shifted, causing the leg on the east side of the conv. to strike against the miners lower left leg. No witnessed the accident, the miner is unable to remember the event.
Employee was on the first step of ladder, he slipped due to poor footing on the ladder and fell onto the ground-fracturing left rib.
Injured worker was positioning himself on ladder to hand a tool to a co-worker who was performing maintenance on the cone. The injured worker put his weight against the guard which hadn't been fastened due to maintenance and he fell six feet to the ground breaking his left leg.
EE jammed finger dosen't know how.
Worker was opening lid of Canica to clean. Lid swiveled around, worker tried to catch it, bending wrist back.

SLIPPED AND FELL AGAINST HANDRAIL OF THE CANICA IMPACT CRUSHER
Worker was ripping the floor of the working bench to supply material to crushing plant. When close to edge material gave way under berm and dozer. While trying to right the dozer it overturned and rolled down the side of the old ramp leading to the bench. Worker received bruises, scrapes to body and laceration to scalp.
MINER WAS CUTTING A PIECE OF BELTING OUT TO USE AS A BACKBOARD FOR THE CONVEYOR WHEN UTILITY KNIFE SLIPPED CUTTING RIGHT KNEE.
WALKING ACROSS PIT, STEPPED INTO POTHOLE TYPE DIP, FELT KNEE POP.
WHEN GETTING OUT OF THE CAB OF THE EXCAVATOR, THE EE SLIPPED ON AN OIL SPOT ON THE STEP, FALLING ON HIS RIGHT SIDE.
EMPLOYEE WAS FEEDING THE SAND HOPPER AT THE WASHPLANT USING A JOHN DEERE THE OTHER LOADER OBSERVED THE LOADER PARKED AT THE SAND HOPPER WITH ITS BUCKET IN THE AIR WITHOUT AN OPERATOR IN THE CA B. THE OTHER LOADER OPERATOR OBSERVED EE'S LEGS STICKING OUT OF THE BOTTOM OF SAND HOPPER ON THE CONVEYER BELT AND SHUT POWER OFF, RADIOED FOR HELP.
