EMPLOYEE WAS CUTTING 2 PIECES OF METAL FOR THE SIDES OF AN L-220 VOLO LOADER BUCKET. AFTER CUTTING METAL 1" X 29" X 9" EMPLOYEE REACHED WITH LEFT HAND TO FREE METAL FROM THE PIECE WHICH IT WAS CUT FROM. WHEN IT BENDED SLIPPED FROM HIS HAND DROPPING ON TOP OF RIGHT INDEX FINGER. THE CUT TOOK 5 STITCHES. THE EMPLOYEE WAS WEARING GLOVES.
SHE WAS RUSHING THROUGH OFFICE TO COMPLETE PAYROLL; TRIPPED OVER RUG. SHE HAD PREVIOUSLY INJURED HER KNEE OVER WEEKEND. SHE HAD SURGERY ON RIGHT KNEE FOR DISLOCATED KNEE CAP. SHE IS NOW HAV ING A SECOND SURGERY FOR SAME INJURY.
