Showing all 6Employee was on a ladder painting a pontoon, about 4 feet off the floor. She misjudged how much room she had and fell off the ladder onto her back. Her supervisor told her not to get up immediately, but stay there until he was sure it was safe for her to move. Once they decided she was okay, she stood up.
Checking oil leak in motor, caught left foot on hydraulic hose, fell--catching himself before going all the way down. Knee and back in pain.
EE and another ee was changing screen on plant when an ee was getting in place to nail down the screen his foot was puncture by a broken wire (screen)
floor was wet with moisture (dew) from early morning. EE made a step forward, slipped and fell to the floor. EE fell on his finger, the momentum of his weight from the fall caused the dislocation and cut to his middle finger.
Reattaching chain on fuel pumps to lock up, during this process the upper arm started to cramp and the employee said he heard something pop. A knot in his arm and bruise happened in the process.
