Two people were tightening a bolt on a pipe flange. One used a ratchet and the other a wrench to hold the head of the bolt. When one tightened the ratchet, the wrench holding the head of the bolt twisted down against a flange and pinched the fingertip of the middle finger on the right hand. There was no nail damage.
