While hosing media into the ditch, the mechanic noticed some plastic that was under the media, covering a portion of the grating. They bent over to pick it up and when they straightened up, lifting the plastic, they felt a shooting pain up right thigh and into the lower back.

The turbulizer broke free and the paddle struck the operator's right palm resulting in a laceration that required medical attention. The operator needed 12 stitches.
Employee was using a grinder to prepare surface when a fragment of the wire wheel struck EE in EE's eye. Employee was wearing EE's PPE for the task which included Hard Hat, Safety Glasses, Face Shield and Gloves. Employee used the Emergency Eye Wash station on site and after the rinse EE still felt that there was more in the eye. Sent for medical evaluation
3rd shift loadout operator struck right shin on gate while exiting the top of a hopper car from the ramp. Operator slipped on the ramp causing the fall. Right shin contusion diagnosis on 4/18/25.
Miner was dismounting from passenger side of a tractor trailer. Miner turned to climb down and missed grabbing the handle on the truck. This caused Miner to lose footing and fall to the ground. Miner struck knee on the ground in the fall, causing the tendon to separate from the kneecap.
Operator stepped off slurry car onto a nut that is part of the bracing of a wall. Operator rolled ankle and went to ER on 10/23/24 and was diagnosed with an ankle sprain.

In the process of dislodging wedged forklift forks from underneath a bulk bagger, an operator struck the steering wheel of the forklift with chin, causing a chin laceration with 2 stitches.
1st shift packaging operator was packaging 50 lb bags. While twisting with a full bag employee strained right knee. The operator was removing a full bag from the spout and turning to stack the bag on a pallet on a turn table.
A packaging operator was standing on the platform at the back of an empty slurry railcar when EE lost EE's footing, hitting EE's right arm on the break wheel of the railcar. The operator sustained bruising to the upper part of the right arm. The operator was prescribed antibiotics and a Tetanus shot.
2nd shift packaging operator reported hurt elbow to lead supervisor. There was not a specific incident that caused elbow lateral epicondylitis.

An employee wearing gloves was manually rotating the FD #1 back mixer to inspect the paddles. While rotating the equipment the cord for the radio got caught on a paddle. When they went to untangle the cord one of the paddles pinched a thumb between the housing.
An employee went to clear a bag that was stuck in-between the discharge conveyor belt and idle roller on Roto Packer C, while the equipment was energized and still moving, in clear violation of procedures . When EE tugged on the bag it dislodged and pulled EE's hand and arm into the pinch point. The employee received bruising to EE's hand and arm.
An employee was hit on the back of EE's right shoulder by a guard beam for a rail loading dock. The injured employee was in a crouched position tending to a portable generator located adjacent to the beam, when another employee inadvertently dislodged the guard, causing the north side of it to fall -12 inches onto the employee below.

The employee attempted to fix it alone by tugging on the belt with left hand, while at the same time 'jogging' the belt with the manual button using right hand to try and 'walk' the belt back onto the idle pully.
