Employee had to stretch to get a plastic cover over a super-sack; in doing so, the employee slipped and fell. Upon impact, the employee's left thumb was jammed into the side of the pallet. The employee was able to stand up but was unable to move EE's thumb without pain.

Showing 50 of 159On 06/23/2025, a 54-year-old employee with 7 years of mining experience was working in the mill near screen 2 packing off bags of finished material When picking up a bag to place it on the pallet, EE felt a strain in EE's back and told EE's on-shift foreman, stating EE's back was bothering EE. The employee later informed the foreman EE was going to the clinic.
While feeding material into a material bin with a feed screw. the employee used a shovel handle to push the material into the feeding screw. The feed screw grabbed the handle and the handle hit the employees face and the employee received a cut that required first aid. There were no stitches; however, a 7 day cycle of antibiotics was provided.
Employee was packaging 50lb bags only 2-3 bags, happened at beginning of shift. Employee stacking height was 4.5 feet. Employee stated EE was swinging bag into place on the pallet.
Employee was walking to locker room at the end of EE's shift to go home. Incident occurred by the Sweco/Duavac in Building #02. Employee was walking and felt a sharp pain in left knee and lower back making EE unable to walk on left leg.
Removing drive pulley off of Jet Mill #01 classifier, using hydraulic puller to remove pulley from classifier off of JM#1. While coworker was applying more pressure to hydraulic puller; puller stripped/kicked off to the side, catching employee's right hand.
Operator was bagging pallets on Screen 5, picked up pallet to place under new sack and felt pain in left shoulder. Continued to work through shift. Did not report to foreman until pain persisted through to new shift next day that started at 7am. Went to foreman at 11am due to pain not subsiding and affecting work.
At roughly 4:30pm on July 22nd, 2024, an employee with a little over 6 years of mining experience tripped on the 2nd step as the EE was ascending the stairs in Shed 8. EE fell on the stairs injuring knee, shortly after which the employee left work and went to the doctors where EE was diagnosed with a contusion on knee.
Operator was feeding raw material into a feed bin when a small granular piece of graphite flung out of the feed bin striking the operator in the eye. The operator was wearing prescription glasses at the time of the accident. Operator requested a hospital visit due to pain in the left eye.
Employee was climbing up ladder outside screen #5 to check bin for fullness with headlamp, when climbing down ladder, employee felt sharp pan in left shoulder. Ambulance was called at 9 pm and was taken to emergency room to be evaluated.
Was doing a screen change on Screen #1 and bottom screen needed stretch out and employee left middle finger popped.
Employer was notified that there were 3 msha reportable audiogram shifts during annual medical review bus. Employee will be informed of shift/results and determine next test to proceed.
Received notification of three reportable MSHA audio shifts and must be reported within 10 working days. Will notify all three employees and those will be re-tested for determination and acquire more information and any steps proceeding.
Employer was advised that there were three MSHA reportable audiogram shifts per annual med bus testing. Employee will be informed and any future testing may be advised.
After annual audiogram testing,1 employee had a reportable hearing loss in their left ear.I reviewed the Hearing Conservation Program,retrained,refitted and offered further evaluation-the employee noted they were having issues with their left ear after a recent day of swimming.Per guidelines,the HCP review was still completed,employee denied further care and evaluation offerings.

Employee was operating a stretch wrap machine when the material they were wrapping became unbalanced toppling over and pinning the employee against a nearby railing. Employee requested medical attention, ER doctors report minor swelling on employees back.
Employee was changing socks on a dust collector, during the process excess graphite built up on employees hard hat. While exiting the job, dust broke free from hardhat and entered employees left eye. Safety glasses were worn at the time of accident.
A maintenance employee was working on Roller Mill parts in the maintenance shop. The employee flipped over a cross member and caught their thumb between the nut on the cross member and the end of the drive shaft sitting nearby. This crushed the employees thumb and caused a severe laceration. The employee was taken to the hospital for treatment and received 9 stitches.

Employee placed a pallet of material on stretch wrapping machine off centered. As the pallet rotated around the wrapping machine it grazed the top of the employees foot due to overhanging the wrapping platform. This caused a contusion on the top of right foot, employee was placed on restricted duty due to injury.

Employee was working around the sack hanger when they missed a step coming down the stairs causing them to fall and land on their bottom.
Employee was mounting and dismounting a forklift to perform normal tasks throughout the shift. The repetitive motion of getting on and off the forklift caused an assumed sprain in the employees right foot.
Employee was driving a forklift with their forks lowered to ground level when they struck a raised section of flooring. The forklift came to halt which cause whiplash type injuries. Employee was wearing a seat belt at the time of the accident.
Employee was pushing a 55 gallon drum of graphite off the fork of a forklift to discard the drum into a hopper. While pushing the drum, it tipped backwards which pinched EE's finger between the drum and fork causing a laceration on EE's left middle finger. Employee received 9 stitches and is expecting to be on light duty for 12-14 days.

Employee was walking from the lab back to production where they slipped on a rock that was on the pavement. This caused them to slip and fall, employee rolled/sprained an ankle.

Was getting off forklift at roller mill 1 when right knee popped. Fell onto right knee after it popped.
Went to step up on forklift and right foot slipped off of forklift. Grabbed handle to keep from falling, twisted, and felt a pop in the right side of back.
Employee slipped and fell near the packing station. Employee broke a rib. The accident was not reported and we were not made aware of this until we received a doctors note on 10/12/2020 pertaining to the employees absence. Accident is still under investigation.

By compactor, employee was pulling a box full of junk, box ripped and employee fell backwards. When Employee fell backwards employee jammed left wrist. Employee was given a velcro splint to wear for two weeks.
Employee was helping to position a gear box onto a screw conveyor when the gear box fell from the chain fall it was being lifted by contacting the employee on the left thigh.
Locker room at Asbury; felt something in right eye. Used eye wash station and also the squeezable tubes of saline solution to try and wash out right eye. Couldn't seem to get out was was in eye.
