Walking up a ramp to loader. Their right foot slipped, they tried to recover using left foot, and left knee bent backwards. The doctors believe they sprained their MCL on left knee. MRI is scheduled for 04/24/2025 and they have a follow up appointment on 04/30/2025 to review the results of the MRI.

Showing all 26Dump truck driver slipped on black ice approaching their truck. When they landed, they broke two bones in their right hand.
Preparing to change bearing on 6x20 shaker. The fork of the Gradall fell and hit employee in the head and EE fell to the ground. There was a cut to the back of EE's head and two compressed vertebrae.
Employee was standing on a ladder cleaning a pin to put bushing in tailgate of the Terex TA400. EE was stepping down the ladder, and was further up than they realized. As EE was stepping down to concrete floor, they ended up falling to concrete floor, landing on the left hip - braking a hip bone, with possible injury to shoulder, elbow, head, fingers.
EE was sitting on loader waiting for another truck to be loaded. He broke out in a sweat and had severe chest pain. He had a heart attack.
EMPLOYEE WAS CHANGING OTR TIRE USING BOOM TRUCK, HAMMER, BARS, JACK STANDS. HE WAS PICKING UP A FLANGE FROM THE GROUND, WEIGHING 80-90 POUNDS, AND PLACED IT ON A CAT LOADER
Employee was grinding. He felt a hot piece of metal on his forehead above safety glasses. When he wiped it off, it fell in his left eye.
Climbed up ladder to remove hydraulic hammer from a track hoe. He stepped off ladder into mud hole, onto loose gravel and turned ankle and leg.
EE was climbing up on 345B track hoe to put fuel in it. He heard a pop and felt pain in his left calf muscle.
Steer cylinder rod was against chest while bent over to pull cap up rod. He felt chest pop, hurt left ribs.
Employee was closing tailgate on dump truck and strained back.
On 6/17/09 EE was bush hogging when he ran into the ditch causing the steering wheel to jerk - jarring his right shoulder. He continued to work until Dr scheduled surgery on 8/27/09.
While employee was climbing down off trackhoe, he felt pain in his right shoulder.
Routine maintenance at crusher - repairing conveyor belts, shim the jay - maintenance.
Employee was loosening a bolt with a wrench to remove a bucket from loader. He used his hand to hold up bolt, spun and cut hand - right palm - had 13 stitches.
Employee was pulling trackhoe teeth off truck. He strained his groin.
EE was reaching for a water hose to wash a bucket and got bit by a spider on his left middle finger.
Employee stepped up on the D25#7 Water Truck. His foot slipped off the truck and he used his left hand to grab a bar on the water truck to keep from falling, which caused pain in his left shoulder.
Ee was carrying a roll of rubber legging to conveyor to replace the legging on conveyor. The legging weighed approx. 10 to 15 lbs and he carried it approx. 20 ft to conveyor. His back started hurting.
EE was swinging a 12 lb sledgehammer to remove a bucket pin from a bucket. He felt a strain or pull in his left elbow.
Employee lifted a screen off the Gradall to put it on the shaker. As he was raising the screen to put it on the shaker, he almost dropped the screen. Bracing the screen to get a better grip, it hurt/twisted his back.
EMPLOYEE WAS WIPING DEW OFF THE BELL END DUMP WINDOWS. HE REACHED BETWEEN THE MIRROR & THE CAB. HE HIT THE TOP OF HIS HEAD ON THE MIRROR BRACKET. HIS HEAD WAS CUT & STITCHES WERE REQUIRED.
EMPLOYEE WAS REMOVING A GEAR FROM THE PIONEER JAW. THE GEAR SLIPPED FROM HIS HAND AND HIS 5TH FINGER WAS PINCHED BETWEEN THE GEAR AND I-BEAM, BROKE FINGER.
WAS WALKING HOSE FROM WATER PUMP TO LOCATE LEAK. STEPPED OVER ROCK, FOOT LANDED ON ANOTHER ROCK THAT SLID CAUSING HIM TO FALL.
HE WAS GRINDING A FRESH CUT ON THE 8X24 BOX ABOVE THE CHUTE, PREPARING IT TO WELD. WHILE HE WAS GRINDING A PIECE OF STEEL FLEW UP AND DEFLECTED OFF HIS CHEEK UNDER HIS SAFETY GLASSES AND INTO HIS EYE.
EE WAS WELDING ON THE PIONEER FEEDER. THERE WAS ANOTHER EE BEHIND HIM GRINDING AND SOME OF THE DUST BLEW UP UNDER HIS WELDING HOOD AND GOT IN HIS EYE.
