Miner was using a handheld grinder on a weld when metal debris flew into left eye. Eye wash station was unsuccessful at remedying, so miner was taken to urgent care for treatment. Doctor successfully removed metal debris with tweezers.

Electrician had the dredge control panel propped open. The prop failed causing the panel to slam down striking EE's left arm just above the wrist. Urgent Care assessed injury as a contusion with no broken bones and directed right handed duty only until follow up appointment to reassess the injury.

While installing a timing belt on a screen, the counter weight came down pinching employee's middle two fingers on left hand between counter weight and flange. Diagnosis: Open fracture; injury of nail bed of finger.

Employees were moving a divider box with a front-end loader and chains. One of the chains broke and stuck the employee in the arm.

The employee spent the day doing various maintenance jobs welding and grinding. EE experienced eye irritation and thought it was due to smoke or dust. Late in the evening, EE went to the ER and had a small piece of steel removed from EE's eye. EE reported the injury to EE's supervisor on 4/8, the supervisor forwarded it to the safety department on 4/18.

Employee was adjusting a feed chute to prevent spillage. The chute is suspended by two chains. When the employee loosened one chain, the chute swung over and struck employee in the head. Employee had left hard hat in the truck and was not wearing it.

Employee was assisting with maintenance activities. EE was lifting a pulley shaft out of the bed of a truck and dropped it on their foot.

Employee was traveling in the basket of a manlift when something caused the lift to unexpectedly increase in speed, throwing the employee off-balance. EE struck EE's ribs on the handrail of the man basket.

employee used a cigarette lighter instead of a striker to light a cutting torch and burned EE's hand.

EE was driving a haul truck when EE struck a pothole in the haul road and reported back pain. EE left work at 11:00 am, called off the next two days and saw a physician on 07/14/2020.

Employee was using a utility knife to cut rubber belting. The knife slipped and cut the employee's leg. Stitches were required to close the wound.

Employee was descending from an excavator after fueling it. Employee's foot slipped off the steps. One hand was on the railing, which was left supporting all of the weight of employee's body, causing an injury to employee's shoulder.

Employee was cleaning out trailer and somehow fell and fractured wrist. Accident was reported to management on 4/10/17.

Delivery truck driver was climbing over the side of the truck box to enter the bed when ee missed a rung on the built-in ladder inside the box and fell, suffering a fractured rib.

A storm was coming in and the wind had picked up. Employees were cleaning up the work site in preparation when one employee had a bit of sandblasting sand blow up under his safety glasses and into his eye. Flushing with water did not remove it so he saw a physician who removed it and prescribed eye drops.

3 employees were assembling conveyor sections to extend the field run. One section was not lined up properly. An employee attempted to pull it into alignment by hand & felt pain in his abdomen. He didn't immediately seek treatment. He later saw a doctor & was diagnosed with a hernia & will be scheduling surgery. He reported the injury on 5/23/14.

Employee was grinding steel. Wearing safety glasses but no face shield. Material got into his eye.

Employee was cleaning the spirals of a sand screw with a wire grinding wheel. A wire from the wheel broke off and struck his eye.

The employee was using a hammer to dislodge a rock from a grate. A rock fragment broke off and stuck into his hand between his index and middle finger.

The employee was lifting bags of cement and felt pain in his back. He did not seek medical treatment at the time of the injury. On 10/14/2010 he saw a physician and was told to take a leave of absence to have back surgery.

Employee was using a bar to lift a feeder into place. He strained too hard and injured his groin.

Employee was walking across the floor of the maintenance shop when he tripped on a gutter grate and fell down.

While driving a haul truck, the employee hit an overhead obstruction. The impact caused injury to his ribs. The employee did not initially lose any work time, but has since decided to retire.

While using a utility knife to cut rubber conveyor belt, the knife slipped out of a groove in the rubber causing a laceration to the employee's left thumb. The wound required three stitches, x-ray and tetanus shot.

Employee was guiding a pan onto a lowboy trailer when he fell through a hole in the trailer bed, injuring his knee.

EE was exiting his loader and twisted his ankle.

EE WAS STANDING ON A CONVEYOR, REACHING AND EXTENDING HIS BODY TO LIFT CHAIN FALL TO JIB CRANE OVER THE CRUSHER BASE. THIS INCIDENT OCCURRED IN FEBRUARY, BUT THE EE DID NOT SEEK MEDICAL ATTENTION OR TAKE TIME OFF FROM WORK UNTIL MARCH 25TH.

DRIVER WAS WALKING FROM HIS TRUCK TO THE SCALEHOUSE. HE FELL BETWEEN THE SCALE AND THE END OF THE SIDEWALK, HIS LEFT FOOT WENT THRU THE SIDEWALK.

WHILE FUELING LOADER, THE HANDLE ON THE NOZZEL STUCK, EMPLOYEE TURNED AROUND TO SHUT OFF PUMP, TWISTING BACK AND HIP.

EMPLOYEE WAS PULLING WIRE THROUGH A CONDUIT IN THE SHOP, THE WIRE BROKE AND THE EMPLOYEE FELL ONTO HIS BACK.

EE WAS CHECKING THE WASH PLANT BEFORE START-UP, CLEANING THE FEED CONVEYOR AND ATTEMPTING TO REMOVE A PIECE OF WOOD FROM THE BELT. THE EE PINCHED HIS FINGER BETWEEN THE SIDE OF THE HEADSTOCK & THE FEED BOX.

EE WAS WALKING ON A HORIZONTAL MANITOWAC BOOM TO PERFORM MAINENENACE DUTIES. HIGHLIFT OPERATORMOVED BOOM CAUSING EE TO LOSE BALANCE & FALL TO THE GROUND. HE HIT LOWER RIGHT BACK & LEGS ALONG WITH HIS CHEST & ELBOWS. MOST OF THE DAMAGE WAS DONE TO THE RIGHT SIDE, THERE ARE NO BROKEN BONES
