Employee was working in a pump house and stepped down on a 3 foot long 6x6 wood board. The board was not sturdy, causing them to fall onto right shoulder, injuring right shoulder and arm.

Showing all 6EE was ascending plant stairway and rolled ankle and fell while turning to descend stairway. Left knee sprain with effusion. Left ankle sprain, restricted to sitting work only. On 12/2/09 had MRI, medial collateral ligament injury to left knee, recommendation of physical therapy.
EE was assisting in removing gravel plant chute and right hand index finger was caught between chute and angle iron. Finger was broken and lacerated.
Operator was cleaning around plant with a loader, while traveling with a bucket of material, went over a bump in the path way and felt something in his lower back pop.
EE was replacing a valve assembly on station 9. lost his footing landing on his right hip against the inside wall of the classifier. Upon follow up discussion with EE it was learned the accident occured when he was getting into the classifier tank.
EMPLOYEE WAS ASSISTING ANOTHER EMPLOYEE IN MOVING 7" LENGTH BY 10" PIPE. PIPE SLIPPED AND FLIPEPD UP AND STRUCK EMPLOYEE IN RIGHT SHIN. PIPE WAS ON DREDGE.
