EMPLOYEE WAS STANDING ON THE NORTH SIDE OF TAIL PULLEY RESTING FOOT ON THE TOE PLATE, UNDER THE TAIL PULLEY SUPPORT FRAME. BELT TRAVELING UPWARDS CAUGHT THE TOE OF THE BOOT AND PULLED HIS FOOT INTO THE STRUCTURAL IRON.

Employee was lifting a bucket of material when he complained of a pain in the neck area. This condition is being treated by his personal doctor and has been recommended for surgery. The condition is a result of a pinched nerve in the neck area. The condition is not a result of an accident. Report is for record only.
FOR RECORD ONLY. Employee of Uvalde Concrete Company picked up a load of cement in transport truck. While on our property he said that he was having trouble breathing and did not feel good. EMS was called and the driver was taken to a hospital and admitted for heart surgery. There was no accident and the above was NOT our employee.

Completing port hole inspection on the pre heat tower. Restriction was found right below port on south 3C. Cleared blockage and went to 4th south level to check port. Opened port & within 15 seconds, material shot out causing burns. Material blew under face shield & hard hat.

EE grabbed the hand rail to catch himself and pulled the tendions in his right arm. The condition worsened with time and required additional medical follow-up and resistricted duty.

Employee was installing a feed chute at the #1 finish mill throat. Unit was being hoisted up by an air tugger. The cable had an overlay wrap off track when chute was being lowered, the wrap of cable let loose and weight of the feed chute shifted to employee, causing a lumbar strain.

Employee was found at bottom of stairs on ground level in office stairs; cause of alleged accident is unknown. EMS was called and employee was given a clean bill of health and released. Employee's statement: fall resulted from a shove. EMS never treated EE. The local fire dept. rendered first aid; the assesment was elevated blood pressure and heat stress.

EE had filled a pill press with sample material. He installed the top cap but noticed it was cocked to the side. When he reached to correct it his lab coat inadvertantly caught the power handle causing the machine to activate. This spewed the sample material into his eye.
Utility personnel dispatched to inspect high-temp alarm on L-23 belt conveyor. When they arrived, the conveyor belt was on fire dropping burning rubber to he ground starting a grass fire.
