EE was exiting skid steer inside the maintenance garage. EE tripped and fell forward onto their hands and knees. The EE's knee hit the concrete floor first and felt a twist in their knee. EE had pain in that knee and was taken to urgent care. EE was given a work restriction of no kneeling or squatting for 5 days.

Showing all 5EE twisted knee while performing maintenance work on a screen (no report filed at this time. A couple days later, EE felt it "pop" again and was unable to get out of the loader due to the pain. Incident was reported at this time and ee sought medical treatment. Was informed on 7/11 that EE had been put on work restrictions.
On 12/20 injured was using a torch to cut bolts on the wash plant. While cutting a bolt, a piece of slag entered injured's left ear. Injured reported the incident on 12/20 but did not request treatment until 12/26.
Injured was changing a tail pulley bearing on a conveyor belt when a prybar slipped off the shaft, causing the shaft to drop onto EE's hand.
EE was removing a suction pipe from a slurry tank. When EE lifted the pipe, EE strained EE's right shoulder.
