While assisting a subcontractors employee experiencing a medical emergency employee felt pain in lower back. Employee tried to manage the pain by stretching and doing at home exercises. Employee requested medical treatment on 11/17/2025.

Employee was descending stairs from the crushing plant control room when EE slipped and hit EE's elbow on the stairs as EE slid down.

Load was dumped into hopper when worker saw a piece of rebar that needed to be pulled out. When he reached to pull the rebar out, a piece of concrete rolled down the wing wall smashing his left thumb between concrete and wall of hopper.

The laborer was under a moving conveyor; when he reached up the belt caught his hand and pulled his arm between the return roller and conveyor belt pressing his forearm against the frame of the conveyor.

A stick fell over the top of the cone crusher, the operator left the control room to remove the stick and a rock struck the back of his right hand.

A return roller was built up with mud and the EE tried to clean it, while it was running, with a bar. The belt grabbed the bar and pulled it up against the conveyor frame, smashing the EE's thumb.

Employee opened the hatch door, failing to latch the door open and standing partially through the hatch, it swung closed, hitting his head. Employee was not wearing his hard hat, did not mention any extreme pain to the plant operator whom he spoke to at the time of the accident. doctor diagnose concussion.
