Stepped on nail while putting scrap in bin.

Employee tried to access door on crusher without proper fall protection, in an area requiring fall protection. Employee fell and injured right hand.

Employee was backing truck on stock pile to dump, positioned truck at an angle, upon lifting bed the truck tipped and landed on driver side.

Employee was trying to pry a rock loose from tail pulley. Pry bar slipped or dislodged and struck employee in left side of face and head. Hard hat and safety glasses being worn.

Lid on WA-500-6 battery box impacted L hand of employee resulting in contusion and abrasion

Heat exhaustion, or symptoms of, related to excessive weather heat event.

Employee was using sledge hammer to loosen hammer shoe in crusher. When struck, the shoe broke and metal shrapnel hit employee in hand and arm. Employee had stitches in right hand and left forearm.

Employee was operating a hand drill, drill bit caught and caused drill to twist and spin. Finger was "jerked" and caused strain. Employee did not notify appropriate management official and report accident until 4/27/2016

Maintained awkward position while welding, resulted in strained knee / sore knee. Knee swelled, developed fluid on knee. Employee reported 5 days later and was treated through a prescription and light duty.

Miner was pushing detachable trailer locking pin back in place. Pin resisted, when pin gave way, finger was pinched.

Stepped out of dump truck, rolled ankle, ankle sprain.

employee was putting new v-belts on crusher, pinched tip of left index and middle finger

Employee altercation - Altercation is being investigated. No witnesses to initiation of altercation. Independent investigator has been hired by Insurance Carrier to review and assist. To my current knowledge Boone County Sheriffs Department is investigating as well.

Employee was power washing equipment (prepping for repair), reached to retrieve something with left hand and accidentally pulled trigger with right hand. Right arm was cut with power washer spray.

Heat Exhaustion

Stepping off ladder and twisted ankle.

Employee slipped while performing maintenance on machinery. Impacted chest.

Lock lodged on machine, when dislodging the rock rolled and struck left hand.

Employee exited loader to inspect shot rock pile, rock rolled from pile and impacted right ankle.

While loading screen cloth from rocks to screen plant, EE pinched finger on loader bucket.

Accidental impact of pry bar during maintenance on crushing plant

Repairing truck scales, decking board pinched finger.

Employee slipped while installing work pump hose. Impacted rib cage on ground and broke a left rib

While connecting a charger to battery of a dump truck, battery sparked and ruptured, sending acid at employee.

Employee replacing bearing on plant. While using hammer, small piece of steel lodged in eye.

Employee slipped on chain ladder and bruised ribs.

Swing gate/chute on plant impacted shins when hinge broke.

Employee accidently sprayed foot w/power washer, hot water resulted in burn.

Ladder slipped, EE fell while working on plant.

EMPLOYEE SLIPPED AND FELL ON HIS LEFT KNEE.

While changing screens, employee felt a pop/pinch in his neck.

Employee slammed brakes on loader, lost power and control, collided with stockpile dump truck (employee statement).

EMPLOYEE WAS BACKING UP DUMP TRUCK AND FELT PAIN IN RIGHT KNEE. CAUSE OF PAIN UNKNOWN.

Employee claims back injury related to loading scrap crusher hammers.

EE WAS ASSISTING WITH THE REMOVAL OF A CYLINDER FROM A WHEEL LOADER. THE SAFETY STRAPS SHIFTED AND PINCHED THE TIPS OF TWO FINGERS ON EE'S RIGHT HAND BETWEEN LOADER FRAME AND PRY BAR.

EMPLOYEE WAS CLEANING HOPPER. LARGE STONE ROLLED AND STRUCK EMPLOYEE'S PRY BAR. PRY BAR STRUCK LEFT LEG FOLLOWED BY STONE STRIKING PRY BAR AND BREAKING LEFT LEG.

EMPLOYEE WAS GETTING INTO DUMP TRUCK, SLIPPED ON RUNNING BOARD & TWISTED RIGHT KNEE. WEATHER WAS RAINY; RUNNING BOARD WAS WET.

EMPLOYEE HAD PREVIOUS FRACTURE IN LEFT ARM. EMPLOYEE HAD STOPPED SEEING HIS DOCTOR AND HAD NOT RE-SPLINTED AS REQUESTED BY HIS DOCTOR. EMPLOYEE DID NOT INFORM EMPLOYEE THAT HIS ARM WAS NOT HEA LED. EMPLOYEE STRUCK A LARGE ROCK WHILE SHOVELING AND RE-FRACTURED LEFT FOREARM.

EMPLOYEE CLAIMS THAT AS HE CLOSED HIS LOADER DOOR, A PIECE OF METAL FLEW INTO HIS EYE. EMPLOYEE DID NOT REPORT INJURY UNTIL AFTER HE HAD WENT TO THE DOCTOR.

EMPLOYEE PLACED 4" GRINDER ON WORK TABLE PRIOR TO WHEEL STOPPING. GRINDER SPUN AROUND & STRUCK HIS FINGER.

STEPPED ON SMALL STONE, KNEE GAVE OUT. THIS WAS AN AGGRAVATION OF PRE-EXISTING CONDITION. THIS DID NOT CAUSE THE CONDITION.

WHILE REMOVING U-JOINT STRUCK CHISEL WITH HAMMERAND PIECE OF CHISEL BROKE OFF AND LODGED IN LEG.

EE WAS INSPECTING AREA AROUND PLANT FOR BUILD UP UNDER CONVEYOR. ROCK FELL OR CAME FROM PLAT AND STRUCK IN HEAD.
