The employee was hosing out tail pulley under pulvomatic crusher and a rock came out of the chute and hit EE on EE's left shoulder.

Employee was walking up side of ramp to see what was wrong with one of the haul trucks and felt a sharp pain in lower back.

Employee was attempting to climb a ladder to access a drain plug on a sand sluice pipe. While climbing the ladder the bottom kicked out causing the employee to fall approximately 5' to the ground, bruising left shoulder and left side rib area. Did not wait for a spotter to secure the bottom of the ladder.

Employee was helping to lift a short conveyor frame when ee felt a slight tug in groin. A follow up exam at a medical facility reveled ee has a hernia.

The employee passed out and hit head on the concrete floor. EE was transported to Olean General Hospital where they were treated for a laceration to head.

The miner was working on reversing the toggle plates in an Austin-Western jaw crusher. Miner was in the rock box removing a cheek plate when the stationary toggle slid approximately ten inches, and during its travel, it came in contact with the miner's hard hat. As a result, the miner received sprained muscles in back and neck.

Used a section of conveyor belting as a floor mat.

He was removing a return roller from the sandstacker. He was using a 9' piece of pipe to push up on the belt in order to take the down pressure off the roller so it could be lifted out of the brackets. he pushed up on the pipe and felt a sharp pain in his right arm between the elbow and shoulder.

This man has a history of diabetes. Anytime he gets a scratch, he gets an infection. He was replacing a bolt in a transmission. He reached down through the floorboard in a D7 Dozer, when he scratched his left elbow. He didn't tell anyone. He got an infection in it and was put in the hospital and on IV's on the 15th of Sept.

Employee was attempting to free a large rock stuck in feed hopper. Had is hand on the conveyor frame when the rock dislodged and fell on his hand before he could remove it.

HE WAS LIFTING A 20' LONG STEEL PIPE OUT OF THE RIVER WITH THE HOIST ON HIS MECHANICS TRUCK. IT WAS COMING UP OK WHEN THE SPEED OF THE CURRENT IN THE WATER PUT TENSION ON THE BASKET AND WHEN IT CLEARED THE WATER IT STARTED SPINNING AND HIT HIM IN THE HEAD AND SHOULDER KNOCKING HIM TO THE GROUND.

Trying to remove a hook roller shaft with hammer, fragment broke off.

OPERATOR WAS APPROACHING LOADER, DUCKED UNDER LAND CONVEYOR & RAISED HIS HEAD PREMATURELY & HIT HIS HEAD. SAID NECK WAS SORE. WENT TO HOSPITAL & DID NOT RETURN. HE HAD WEEKS VACATION. WHEN HE RET'D TO WORK HE SAID NECK WAS STILL SORE. WE SENT HIM TO DR & HE WAS PLACED ON LIGHT DUTY 8/17/04.

TIGHTENING BOLT ON SCREEN DECK. WRENCH SLIPPED OFF CAUSING IT TO PINCH PALM OF RIGHT HAND AND CUTTING PALM OF RIGHT HAND.

EE STEPPED UP ON WELDING TRAILER TO START AIR COMPRESSOR. WHEN STEPPING OFF OF TRAILER HE STEPPED ON STONE AND SPRAINED RIGHT ANKLE.

CAME TO WORK WATER RUNNING OUT GARAGE DOOR FROZEN WATER LINE CAME INTO OFFICE SHUT OFF WATER WENT BACK OUT TO MY TRUCK & SLIPED ON ICE FROM THE RUNNING WATER.

PUTTING TEETH ON DRAG BUCKET STRUCK END OF TOOTH WITH HAMMER A CHIP OF STEEL STRUCK HIM RIGHT OF HIS RIGHT EYE. HIS SAFETY GLASSES SLID DOWN WHEN HE SWUNG HAMMER.

SHOP DOOR STUCK. TRIED TO PUSH TOP OF DOOR TO OPEN IT AND NAIL STAPLE WENT INTO LEFT WRIST (TRIMWAS MISSING AROUND WINDOW). THE SHOP DOOR HAS BEEN TAKEN OFF.

EE WAS SITTING ON PLATFOERM, GRINDING A CHUTE WHEN THE PLATFORM FELL OUT FROM UNDER HIM AND HE FELL 10' INTO BIN. (COMPLETED AFTER TALKING WITH INSPECTOR).

HE WAS CUTTING A PIECE OF BELTING WITH UTILITY KNIFE WHEN IT SLIPPED AND CUT LEFT FOREARM.

CUTTING JUNK BEAMS WHEN SPARK WENT INTO EAR. WENT TO DOCTOR ON 3/24/00.
