Sunny, cold day, light winds, ambient temperature approximately 15 degrees F. Subject slipped and fell while entering a truck. Surface of the ground was packed snow with small gravel sprinkled over it to aid traction. Damage according to physician was fractures to ribs and vertebrae.

Employee was putting air into loader tire when metal ring which holds the tire in place failed and flew off the rim. Employee sustained injury to hip and pelvis. I am unsure of exact nature of injury at this point. I am also not sure what hit the employee causing the injury.
