Employee slipped while walking out of enclosed trailer.

EE felt pain in shoulder after slipping on stair tread. Injury was deemed reportable on 11/11/2024

Employee started feeling knee discomfort and unstableness while pulling cords during equipment teardown. Employee was prescribed physical therapy for string band strain. Injury was deemed work-related on 10/01/2024.

While securing a hydraulic line that was not in use it slipped and contacted the employee's mouth. This resulted in a small chip to a tooth. EE was seen at a dental office on 9/19/22 where it was determined to need repair.

A left ankle sprain occurred when the employee stepped on a four inch rock and rolled their ankle.

Pinched and lacerated left middle finger tip installing roller gang on conveyor - laceration required stitches.

Slipped and fell at same level while splicing a conveyor belt - lacerated left inner thigh on angle iron.

Bent over at waist to pick-up a chain and strained lower back

Accidentally stepped on lump of frozen sand and rolled ankle.

Preparing conveyors for transport, received small laceration near right knee on extruded metal edge of conveyor guard while walking by.

Miner felt pain in left foot and ankle after prolonged standing on ladder.

Miner fell from tire of Cat Motor Grader, landed on back, fractured rib. Miner reported accident, but did not report injury until 07/24/2017.

Miner was climbing down right side ladder of Cat 980 loader, missed step and fell to the ground.

Miner stepped off of a middles rung of a ladder thinking he was at lowest rung. The impact with the lower level injured his right hip and thigh.

Removing sand from malfunctioning truck tarp, spring loaded tarp arm struck miner in face when enough sand had been removed to release tarp.

Miner was stepping out of skidsteer and lost balance and fell twisting ankle, knee and hip when landing.

Employee fell from a ladder approximately ten feet, landing on feet, but injuring his back.

Recordable hearing loss discovered during annual audiogram.

Miner was using a utility knife to cut belting, the knife slipped and he cut his left thigh, which required sutures to close.

Repetitive motion occupational injury - ulnar nerve lesion - right forearm, causing numbness and pain in right forearm. Probably caused by shoveling.

Miner tripped on top step of stairs leading into generator trailer, he put out his hands to catch himself and broke his left wrist when his hand contacted the floor.

Individual intially reported injury to self at home. Later investigation revealed on 01/07/2010 that employee was injured at work. Employee injured back (3 crushed vertebrae) upon falling from bank/berm of silt pond.

Miner was hammering on a bucket on a dragline bucket, when a piece if iron broke off striking him on the chin.

EE was entering a skidsteer loader when he slipped on the step and cut his left leg. stitches were required.

EMPLOYEE WAS STANDING ON AN I BEAM 4' OFF THE GROUND LIFTING A BELT WITH A VICE GRIP WHEN HE SLIPPED AND FELL TO THE GROUND INJURING HIS LEFT WRIST.

EMPLOYEE WAS CUTTING A CONVEYOR BELT WHEN THE KNIFE SLIPPED AND CUT HIS LEFT LEG ABOVE HIS KNEE.

EE WAS CUTTING THE TIE WIRE USED TO SECURE PENDENT LINES DURING TRANSPORT OF THE CRANE. WHILE CUTTING THE LAST TIE WIRE, THE PENDENT LINE SLID AND HIT HIS ARM.
