TO ENTER DUMP TRUCK EMPLOYEE NEEDED TO STEP UP A COUPLE OF STEPS ON THE TRUCK. HE ATTEMPTED TO PULL HIMSELF UP BY REACHING UP AND GRABBING THE STEERING WHEEL TO HOIST HIMSELF UP. HE FELT A P ULLING IN HIS SHOULDER AND A POP IN HIS NECK. (LEFT SIDE) PINCHED NERVE SWOLLEN VETEBRAE.

OWNER WAS LEAVING THE SUPPLY VAN AND MISSTEPPED THE STEP DOWN, A SLIGHT DROP OF APPROXIMATELY 8". HE PITCHED FORWARD AND LANDED ON HIS KNEES. HE RUPTURED THE TENDONS IN BOTH LEGS.

WAS CLEARING BRUSH FROM QUARRY AREA. HE WAS USING A END LOADER TO CLEAR THE BRUSH BUT GOT OUT OF THE END LOADER TO PICK SOME WOOD BLOCK HE GOT HIS FEET CAUGHT IN SOME BRUSH AND SLIPPERY SNOW. HE TWISTED SIDE WAYS AND FELL TO THE GROUND ON HIS PSOTERIOR END.

ACCIDENT HAPPENED DURING PREPERATION & SET UP TIME PRIOR TO ANY ACTUAL OPERATION. 2 WORKMEN WER LIFTING A HEAVY PIECE OF METAL. UPON LIFTING IT THE END HELD BY INJURED EE WAS FORCED BACK AGA INST THE N EXT PIECE OF METAL SHEET & HIS FINGER WAS PINCHED BETWEEN THE 2 PIECES.

EMPL OPERATING BREAKER CRUSHING ROCK.LARGE ROCK CAUGHT UP AGAINST ANOTHER ROCK AND WOULDNT GO THROUGH JAW.IT SUDDENLY JUMPED AHEAD AND PINCHED OPERATOR FOOT AGAINST PLATFORM.
