Very wet and muddy, had been raining previous two days. Employee was stepping off of flatbed service truck, slipped and landed on trailer hitting right side rib cage on trailer jack. resulted in a broken rib and deep contusion to EE's right side.

Showing all 8Equipment versus employee vehicle, no injury, damage to vehicle only. Daylight, no adverse weather conditions. Damage to driver's side door of employee's vehicle. Damage estimate of $3528. Equipment operator did not see emplyee's open door and caught door with edge of bucket.
Employee was cleaning beside crusher. A rock rolled over feeder edge and fell striking employee in shoulder. Do not know extent of injuries at this point. Update: Shoulder was bruised, no broken bones.
Employee was exiting vehicle and slipped and fell. Caught self on ground with right arm and broke right wrist.
Employee was changing bucket on skid loader. He had it lifted off the ground and it fell landing on his ankle.

EMPLOYEE WAS HELPING SET UP TRUCK SCALES AND GOT FINGER PINCHED BETWEEN 2 PIECES CAUSING A CUT THAT REQUIRED STITCHES.
EE WAS CLEANING UP OVER SPILL ROCKS BELOW CONVEYOR BELT. PICKED UP ROCK & TURNED & FELT PULL IN LOWER BACK
