Employee was repairing the mounting of the engine compartment door on 937 light plant. The door was on the ground. He then lifted the door to get ready to install it. He turned his body with the door and wrenched his back.

Showing 50 of 172While coming down from changing breathers on hyd tank, employees left foot slipped from bottom step of the ladder. He landed on the edge of his foot causing it to twist to the left. He was taken to the hospital and given a full duty release but on a follow up exam (05/06/08) it was discovered that he had soft tissue damage and was put on Restricted Duty.
After taking a break at Mine Ops, the truck driver noticed a pain in her right shoulder after boarding her haul truck. After EMT checked her out she was sent to NNRH for an evaluation. She was placed on restricted duty.
While stepping out of the cab of the lube truck his foot slipped off the step. He was holding the inside door handle. As he hit the ground he heard a pop in his shoulder followed by a throbbing pain. He was taken to the emergency room and was given a full duty release. On a follow up visit on 3/26/08 with the doctor he became a restricted duty injury.
Employee was assisting loading blast holes when he fell backwards into a 4 foot deep hole. He remained working for about one hour, and then collapsed. He was diagnosed with a massive concussion.
While employee was walking ground gave way and employee fell backwards into a hole and left ankle was caught between two rocks and twisted
Employee was removing a flowmeter form the south facility pumphouse. He rolled the flowmeter to remove and rolled back, and the tab on the flowmeter cut his right wrist. 9 stithces were required to close the wound.
Employee was moving pallets in the laydown yard and felt a dull pain in his lower back. The next day at work he was moving pallets again and the pain became worse. He was seen at the emergency room and placed on light duty.
While tripping drill pipe, employee pulled back on the top head to allign pipe. As he pulled he felt something tear in his left shoulder. Doctor placed him on light duty pending the results of an MRI.
Employee was climbing on a road grader and when he went to pull himself up he felt a pop in his right shoulder and he felt a pull in his muscle. He went for a follow up visit to the Doctor on 02/14/08 and was put on sedentary duty and was scheduled for surgery. Became a lost time on 02/20/08.
Employee was walking between core tables and to avoid some water she stepped over to the right side. The grate was sticking up a 1/2" above the others and her toe caught on the grate. She tried to catch herself as she was falling but she twisted her body and fell on her right side. She went to the doctor on 5/27/08 and this was when it became a Lost Time.
Changing bit subs, caught finger between lifting bell and winch line hook. This resulted in loss of fingernail and 4 stitches on the middle finger of the right hand.
He was making room for liner and lifters and while rolling a 55 gal drum he aggravated his shoulder. He went to the doctor on 01/16/08 because his shoulder was still bothering him and got a full duty release. Went back to the doctor on 02/20/08 because it was still sore and was put on restricted duty.
Helper was running out drill pipe onto pipe truck and strained a muscle in the middle of his lower back.
He was hooking up the mobile steam cleaner to service truck. When he released the trailer jack it retracted instead of dropping to the ground as expected. He was using his left hand to push against the jack while pulling the pin with his right hand. It pinched and cut his left middle finger. His wound was cleaned up and then he was taken to the emergency room at NNRH.
Employee was being loaded with lime at the south lime silo when he noticed dust in his cab. First his nose and sinuses began to burn and then his mouth, throat, chest, and eyes. He was taken to NNRH and held for observation.
Mechanic cut hand while removing tie wrap
Employee slipped on ice and fell to ground fracturing a bone in his right foot.
Installing splits in inner tube. Pushing with gloved hand, split stopped, hand slid up the split cutting thumb.
Employee was climbing ladder to empty dust pots and slipped. He spun around and slid down the ladder. His arm got caught on a bolt causing a laceration.
While the bus was loading with employess, a co-worker set in the seat behind me and shoved the seat forward; it was in the reclined position. When this happened I was laying on my right side; this immediately sent a sharp pain from my shoulder to my neck. Qnce we were at the office on mine site I notified someone. The pain is in my right shoulder, into my neck and down my arm.
Employee reported a sore back and the injury was initially treated as a muscle strain - first aid. After the back did not improve, he went to a chiropractor for an MRI and subsequent treatments which began on 8/13/07. At this time the injury became reportable.
The operator was climbing up on his machine and slipped off causing the operator to land on the back of his heel twisting his ankle. He wasn't reportable until a follow up visit on 9/18 showed he had a torn achilles tendon.
Employee was lifting spare tire into the back of a service truck and felt a pop and then intense pain in his lower right back.
Employee was lifting and carrying truck parts. When the item was being lowered to the ground, the item slipped from the employee's hands and the employee stated she felt like she tore something in her back.
While trying to open the gate on the flume a steel grating that was leaning against the railing fell over and struck employee.
The employee climbed up onto the tie rod on one of the Liebherr T282B haul trucks so he could tigthen an overhead fitting. The employee slipped on the tie rod, lost his balance and fell approximately 4 feet to the ground, landing on his right foot.
Employee was in the Assay Compressor and DI room and he was changing the DI water tank filters. This wasn't reported because of a change in management and was just recently discovered that it hadn't been entered. This was a medical treatment injury with a full duty release.
Helper was placing a drill bit into tool box mounted on side of truck. He miscalculated the distance between the bit and bottom of box, and his left middle finger was pinched between bit and metal tool box.
Single part down haul ball on crane was 2 blocked which caused the cable to be severed. Employee's right hand was struck by falling down haul ball.
He was loading cement at the Cortez Mine laydown yard. Although a fork lift was available he chose not to use it. While he was lifting a bag of cement he pulled a muscle in his back. Weather conditions was clear and dry, ground conditions was dry and level. Placed on light duty as per Dr.
Employee was throwing scrap wood into a dumpster. A 2x4 piece of wood bounced back from the dumpster and hit the employee in the back of the head, just below her hard hat.
Lifted a small heater (about 40 lbs) didn't realize diesel tank was full. Developed lump in stomach area.
Tripping rods in. After connection, stepped back and stepped into deck hole for rods. Fell against stool straining back.
Drill helper was running out pipe on pipe truck in slick conditions and was going slow. Driller was lowering the top-head down, and it caught up with the pipe and struck the pipe end. This event caused the other end of the drill pipe to strike the helper in the groin area. Bruising to groin area.
Individual was on a ladder, tightening a hydraulic fitting when the fitting broke, releasing hydraulic oil. This startled the individual which caused him to step back off the ladder, he then fell to the ground and injured his right elbow and wrist.
when using both hands they will go numb and lose the feeling in them(like hammering, running drill or writing etc. etc.)it's been going on for about 2 years but just reported it to us.
At approximately 9:00am, employee tried to pick up a large wheel chock from the ground. The wheel chock was frozen to the ground which caused employee to pull something in his elbow when he attempted to pick it up.
Driller had back to equipment and was talking on the phone. Helper backed drill pickup up road, struck driller in back and knocked him down. Driller was unable to scramble out of way and right rear tires of truck ran over driller's legs between knee and ankle. Contusions to left leg - no broken bones.
EE was walking towards 505 water truck to work on it when he slipped and fell on the ice.
EE was getting off work she went to load into the Cortez Van when she slipped and fell on the ice causing a strain to her right shouder. Resulting in modified work
While walking up the stairs at mine operations,he reached the top step and slipped on the top of the walk way falling on his right knee and hitting his right shoulder against the building causing(A strain to his right shoulder at the rotater cuff)
Employee was lifting a bucket of flux (30 to 40 lbs). While lifting the bucket she felt something wasn't right and by the next day her back was hurting.

In adverse weather conditions, a driller helper stepped down from a water truck, slipping and twisting his knee.
Employe was washing down in #8 agitator when he slipped in the mud and hit his left hand against a support beam causing a laceration to his L index finger.
