Showing all 8Driver was stopped by a flagman in a highway construction zone; he was hit from behind by a pickup truck causing damage to the rear axle hangers of the truck. Driver has resulting back pain.
EE was lying on his back holding a 1" impact tool, breaking loose nut and bolt on cutting edge. EE had leg wrapped around cutting edge to hold counter sunk bolt from spinning. As bolt came loose, it popped up and spun, cutting through his pant leg, causing laceration to left knee.

Mack dump truck was being used to haul raw material to the grinding room stockpile. EE was exiting the truck when his foot slipped and he fell to the ground (fractured rib).
EE WAS TRYING TO LOOSEN A T POST TO PULL OUT OF GROUND WHEN EE PUSHED THE POST AWAY. EE HAND SLIPPED OFF THE POST AND IT SPRUNG BACK TOWARD BODY. A PIECE OF BARBED WIRE ON THE POST STRUCK EE ARM ABOUT 2 INCHES

The employee climbed up on the loader to wash the windshield. The surface was wet with the early morming dew. He slipped and grabbed at the loader to avoid falling. He lacerated the palm of his left hand.
