EE was walking out of The office and took a bad step. EE fell landing on EE's knee and wrist. EE notified EE's supervisor in which EE decided to go to the doctor to get checked out. The doctor diagnosed a sprained wrist.
The operator was utilizing the reed screen during work shift. At 12:30 pm the operator went to shut the screen off for the day. The operator approached the screen shutting the shake feature of the screen off. In doing so the operator removed their hard hat to get a better look at the hour meter of the engine and a rock vibrated off, hitting them in the head.
Operator was loading haul trucks. Mechanic showed up to address an issue on the loader that had occurred. Operator briefly stepped out of loader to talk with mechanic about issue with loader. Using 3-point contact to step off loader, the operators hand slipped causing EE to hurt EE's knee. Operator did not fall off ladder.
Individual was using an extended branch saw to cut branches on a tree. When pulling the cut branches from the tree they came down on top of them causing a small contusion on their head requiring 3 staples and a small contusion to the right arm. Dr. excused them from work for 3 days and employee is returned with no limitations on 7/26/2022
Equipment Operator was shoveling under a screening plant and noticed the next day that their shoulder was sore. After a doctor looked at it, EE was put on light duty. Operator is now driving a water truck and there were no missed work days.
In the event of a belt change, Maintenance man's hard hat fell off. While retrieving the hard hat, EE grazed a piece of angle iron. EE noticed had been cut on the head. The bleeding was treated with gauze and a rag, then was taken to Banner Occupational Health to receive further check up of the injury. EE received 8 staples and was released to work the next day.
Employee tripped and fell on the front stairs of the office.
Employee was operating a hand held grinding tool. The grinder bounced back off the metal surface cutting the employee's left hand.
The loader operator was traveling across the main yard and struck a pot hole. The impact jarred lower back.
Employee struck a hole while operating a loader. The impact caused the loader seat to bottom out jarring the operator's lower back.
While exiting the crusher 1-02, EE lost his footing and fell on his left side.
Two employees were working on the same screening plant. During the course of making the repairs one employee was exposed to "arc flash" from the other employee's welding activities.
EE was desending the ladder. At mid-point when he stepped down, his weight shifted pushing the ladder to the right. His right foot became entan geled in the step of the ladder.
EE WAS HELPING CHANGE OUT THE SCREEN DECKS OF THE WET SCREENING PLANT. HIS FOOTING CAUSED HIM TO FALL BACKWARD, LANDING ON HIS BUTT. EE DID NOT REPORT THE INCIDENT OR MISS ANY WORK FOR ONE AND ONE HALF MONTHS.
MAINT. CREW WAS REPAIRING A CEDAR RAPIDS CONE CRUSHER. TOP OF CRUSHER HAD BEEN REMOVED EXPOSING A HOLE IN THE WORK DECK, WHERE THE CRUSHER ACCUM. HAD BEEN. THE INJURED PERSON THOUGHT SHE WAS S TEPPING INTO THE SMALL OPEN HOLE & OVER CORRECTED HER POSITION BY REACHING FOR A NEARBY LADDER. A FEW HOURS LATER SHE COMPLAINED OF A BACK PAIN & AT THAT TIME WAS TAKEN TO A MEDICAL FACILITY.
WHEN COMING DOWN LADDER OF THE 988 LO0ADER, EE FELT A PULLING SENSATION IN HIS LEFT SHOULDER.
EMPLOYEE WAS PICKING UP BUCKET OF WATER TO POUR INTO TOILET TO BE ABLE TO FLUSH IT. SHE HURT HERLOWER BACK.
EMPLOYEE WAS LEAVING EAST ENTRY AND TRIPPED ON ROLLED CARPET.
EE WAS GRINDING METAL WELD FOR GUARD WHEN CHIP/SHAVING FLEW INTO HIS EYE (RT.).
EMPLOYEE WAS REAR-ENDED ON BEELINE HWY @ ENTRANCE AREA OF HAULING ROAD.
EMPLOYEE WAS CHANGING SCREEN AND DID NOT TIME RELASE OF HIS SIDE. WAS STRUCK ON LOWER RIGHT ARM BY SCREEN.
REMOVING SCREEN EMPLOYEE HAD LIFTED ONE SIDE OF SCREEN AND DID NOT RELEASE ON TIME, WIRE CAUGHT HIS SHIRT, TWISTING RIGHT ELBOW.
EMPLOYEE CLAIMS HE FELT SHARP PAIN IN LOWR BACK WHILE SHOVELING.
EMPLOYEE WAS STANDING ON A PLATFORM, APPARENTLY LOST HIS BALANCE AND FELL 42.5" TO THE GROUND. ON AUGUST 8, 2000, THIS WAS DEEMED NOT CHARGEABLE BY THE FATALITY REVIEW COMMITTEE.
