Miner was using a sledge hammer to remove a stuck rock from the grizzly bars. Upon the rock coming free, a piece of the rock struck miner in the mouth resulting in a fractured #9 false tooth. Miner is still working and having dental corrections made to the tooth.

Miner stepped out of operator's station of Cat 14H blade to close open engine bay door. In doing so miner stepped onto rear tire that was covered in mud, slipping & falling to the ground. This resulted in spraining left wrist. Miner had MRI completed on 3/10/24 & results were provided to SRMG safety on 3/12/24 stating there was in fact a fracture in the wrist. No other issues.
Miner was trying to remove a rock that was stuck underneath the jaw crusher with a 5 lb sledgehammer. While trying to dislodge the rock that was stuck, the miner strained EE's right shoulder.
Miner was shoveling material around the crusher area. After getting home, EE's back became sore and tight.
Miner was running for cover from the starting rain storm when EE stepped on the concrete near the flocculent tank. In doing so EE stepped on a small amount of spilled wet flock and slipped. Miner said they felt a pain in lower back.
On the morning of 12/6/2022 miner reported to their supervisor that on 12/05/2022 they had jumped from the Jaw crusher frame rail and injured their right knee upon landing on uneven surface. Employee was sent for medical and placed on restricted duty for a sprain knee.
Miner was stepping out of service truck when they missed the bottom step and fell to the ground. Miner impacted the ground with left shoulder resulting in an injury to the shoulder.
Miner was using a swinging sledge to tighten the mantel nut on the crusher. When the sledge swung past the wrench the miner attempted to catch the sledge on its backswing and got finger smashed between the sledge and the wrench. Miner received medical attention for the injuries and stitches for the cuts.
Miner's personal pocket knife came open in miner's pocket and tore through the material of miner's jeans exposing the blade. Upon sitting down the miner ran hand down leg contacting the exposed blade and cutting hand. 3 stitches were given by medical to close the wound.
Miner was climbing into service truck when they felt something shift in lower back. Miner requested medical attention and EMS was called. Miner was taken by ambulance to the ER where they were admitted for lower back strain.
Miner was adding engine oil to a 775G haulage truck. As EE was finishing the task, EE closed the engine compartment access hood/door. EE wasn't aware that EE's right thumb was placed on a pinch point area and struck EE's right thumb causing a laceration. EE was treated and received stitches. Miner was given restricted duty.
Miner was climbing the ladder of a 775 haul truck during servicing of equipment. While climbing miner felt a pull in lower left abdomen. Upon medical review miner was given restricted duty for a strain.
Miner was changing guarding on crusher when miner stepped backwards and stepped on another guard lying on the ground. This resulted in miner twisting an ankle. Miner was placed in a removable boot from medical and placed off work until Jan 18th, 2022.
Miner was cutting urethane crown bar protectors to size with a utility knife. On the final cut the urethane bar shifted and caused the left index finger to move into the blade's path. The knife trespassed the leather glove and into the side of the left index finger. The miner received 12 stitches.
Miner was operating a front end loader and sustained a contusion of lower back when the seat of the loader bottomed out when miner hit a bump. Miner was placed on restricted duty upon medical review of the injury.
Miner was inspecting the head pully of a conveyor belt on the Jaw crusher while standing on the chassis of the jaw. Miner stepped over to one of the tires and it rotated causing miner to start to fall. Miner grabbed the support leg of the conveyer to stop the fall and strained right shoulder and upper arm. Miner was placed on restricted duty by medical.
Miner cut EE's left ring finger while changing segment plates from a John Deere 944K loader bucket requiring 8 stitches. Miner was holding bolt head as EE impacted the nut with the impact wrench spinning the head of the bolt.
While miner was greasing mobile equipment miner went to step back into service truck and felt a pop in lower back. Miner was sent to medical for evaluation as miner requested and was placed on restricted duty due to a reported strain of muscles in lower back by medical.
Miner was removing a 12" stuck rock off the grizzly bars from the jaw crusher when the rock shifted. As a result the rock struck miner's left ring finger cutting the finger requiring stitches to close the wound.
Miner was stockpiling material away from the mortar sand stacker belt when the 988K loader EE was using came in contact with the belt. The belt was pulled onto the loader and the head pulley came thru the front windshield of the loader as it fell. Miner injured EE's left foot when the dash of the loader compressed onto EE's foot. Miner had surgery on foot to correct the injury.
While operating a 745 Articulated Haul truck the left front tire blew out. Miner was parked loading next to a wall that redirected tire blast back towards the cab of the haul truck sending broken glass from the windows into the cab resulting in the miner getting cuts and abrasions to left side and right eye. Miner also sustained unknown temp hearing loss in left ear.
Miner had a personal injury that miner came to work with and while operating the M982 loader it irritated the injury more. Additional strain of right lower back muscle due to improper posture while operating the loader.
Operator was dismounting the 670 G Excavator. Using 3-points of contact, operator's right ankle rolled when placing foot on uneven ground resulting in a sprained right ankle.
Miner was replacing cutting edges on a 980G loader and in the process EE was pulling the center plate from underneath the bucket so it can be picked up with a boom. As the miner released the cutting edge and stood up, EE felt a "pop" in the lower right side of the back. This resulted in a muscle strain in EE's lower back.
Miner was swinging a 20 lbs sledge tightening the locking nut on the cone liner during a liner change. While swinging the sledge they accidentally struck a near-by chute which redirected the sledge missing the nut and struck right foot. The injury resulted in positive x-rays for a broken right foot.
Operator error. Failed to set crane up in digging mode. Employee did not set the travel locks prior to digging.
Employee was installing return rollers when they felt a pop in right groin area. Miner reported it as a first aid only report at the time. On 03/24/2020 the miner requested a medical evaluation due to it not healing. EE was sent to urgent care where they were placed on restricted duty pending a specialist follow up for a hernia.
Miner was climbing down out of the 980K loader during the pre-trip inspection and missed the bottom rung of the ladder that is attached to the loader. This resulted in EE falling and landing on hands and back. 6 hours later the miner requested to be checked by medical. X-rays came back negative and miner was given one off work for recovery by the doctor.
Miner was kneeling on right knee while shoveling material out from under the cone crusher. When the miner stood back up they noticed that the right knee did not feel right. The right knee sustained a strain injury.
At the end of the miners shift on 9/12/2019 the miner felt EE had something in EE's right eye. The next morning 9/13/2019 at work EE's right eye was red and sore so EE requested medical evaluation. EE was sent to the doctors office where the doctor prescribed eye drops to be placed into EE's right eye.
Miner was shoveling under a Jaw Crusher when they strained their left wrist and right elbow. Employee was sent for medical evaluation and placed on restricted duty. Miner returned to work and is currently working restricted duty.
Miner was climbing out of the cab on a 621 G water pull when EE missed one of the steps on the way down. This resulted in the miner falling from the equipment landing on EE's backside and causing a contusion to EE's mid and lower back. Miner was placed on restricted duty by the urgent care center.
Employee was trying to loosen plug on front axle in order to check the oil level and to pull a sample. The plug was tight, so employee was going to shock it by hitting it with a hammer. The employee missed the plug with the hammer and hit finger instead.
While pull starting a chainsaw the miner felt a pop in the left wrist. X-Rays were taken and came back as negative but the miner was placed on restricted duty by the doctor to rest the wrist.
Miner was using a box cutter to open the other packaging of some press bags for the wash plant when the cutter slipped and cut EE's right pointer finger. This resulted in the miner receiving medical treatment in the form of a Tetanus shot and stitches. Miner returned to work the next day on restricted duty.
Miner was walking across an illuminated flat working surface of the mine located at the equipment parking area while greasing the equipment. In doing so EE stepped on a rock which resulted in EE twisting EE's right ankle. Reported the next day and was sent for medical evaluation which placed EE on restricted duty.
Miner strained lower back when EE was laying down on back side shoveling out from under the jaw crusher.
Miner was cutting a piece of 1" hose when the knife slipped off the hose and cut the palm of the left hand under the thumb. Miner required 3 stitches and a tetanus shot to close the wound.
Driver was cleaning side window did not have three points of contact when stepping off the semi. Twisted and broke right ankle on flat surface.
Miner was carrying a stack of five 12" x 12" screen panels weighing approx 25 lbs up the stairs to the screen deck. At the top of the stairs when the miner attempted to raise the panels above their head to place them onto the screen deck they felt a pop in the lower back. This resulted in a strain to the miners lower back.
Miner struck a finger while using a 3 lb sledge hammer and a hole punch to punch holes in flashing while making repairs to the Fat Pak Crusher. This resulted in a crush injury to the left index finger and restricted duty.
Miner was cutting/grinding a 20'x5" Channel Iron when it fell from the work bench catching left glove on the way down. This resulted in a strain to EE's left shoulder when it jerked EE's left arm. Miner has been placed on restricted duty and a MRI of the shoulder is pending.
Scale operator opened the scale window and dust blew into EE's left eye. Employee rubbed EE's eye to remove the dust which resulted in an eye irritation. On 5/3/2017 the employee called "Teledoc" and the doctor prescribed an eye drop antibiotic as a precaution. Employee did not see the doctor and needs no future medical treatment. Employee reported incident to SRMG on 5/3/2017
Team loaded excavator onto trailer for transport. Employee climbed to top of equipment to get a height measurement for permitting. EE was not wearing fall arrest gear. While climbing back down, EE slipped on some hydraulic fluid and fell approx. 16' to the ground. EE's left arm hit the trailer beam, EE bounced off the track and landed on the ground.
Miner was climbing into a IT-28 wheel loader and missed step. As employee started to fall they caught themselves with right arm and felt a pop straining right shoulder. No medical treatment was requested at this time and a report was made. On 4/11/17 miner requested to go to the doctors and per doctors request was placed on restricted duty on 4/11/2017.
6/6/2016 Miner pinched right middle finger between a clamp rail and angle iron while changing screens when EE laid the rail onto the screen box to use later. Miner refused medical treatment and it was reported as a first aid only that day. On 6/8/2016 miner went to the hospital to have fingers looked at and it was reported by the hospital as broken.
Working on the toggle plate at the jaw crusher, miner was using a pry-bar to break the toggle plate loose. As the plate came loose it caused the pry-bar to shift compressing the miners hand against the wall of the crusher. Miner believed EE was ok at that time. Six days later the miner went to doctors office and had it looked at. An X-ray was taken and discovered a broken bone.
