while changing teeth and cutting edges on 988H loader 23.3804 eq #. a wrong bolt was inserted in one of the holes by mistake and while removing it EE cut EE's left ring finger. first aid was rendered and later in the morning went to Urgent Care where they cleaned the wound and did 4 stitches. no restrictions and returned to work same day

Showing all 37EE had parked EE's service truck at the shop. Was disembarking EE's truck using 3 points of contact. Boot apparently had moisture on it causing EE's foot to slip off last step. EE then lost balance and fell backwards in a sideways position and landed on EE's shoulder. Step is in good condition and clean. EE worked the rest of the day and went to urgent care after work
Injured was changing a hose on a Cat 773F from a 6 foot fiberglass ladder. It appears as EE was making EE's descent EE caught EE's pants (rip in pants) on the cover of the quick fuel connector. Causing fall backward, hitting shoulder and head on ground. EE was disoriented but answered basic questions correctly. Back of EE's head was bleeding somewhat, sent to ER to be checked
EE had started investigating possible deterioration to a metal grate in flooring by VSI crusher. While preparing to make repairs the grate gave way and EE fell to the ground approximately 12 feet below landing in mud softening EE's landing. EE refused medical at the time said they were ok. Talked with EE several times over the weekend, talked EE into getting some xrays
EE was assisting a co-worker changing shoes in VSI crusher. After removing the lid and began taking off the top ring, EE's finger was smashed between the ring and one of the shoes causing a cut. The co-worker took EE to the hospital emergency room. Assessed as a broken thumb and laceration of thumb. Stitches required as treatment.
EE exited EE's haul truck at lunch time to share part of EE's lunch with another EE. While walking on the terrain, EE slipped on the partially thawed muddy ground. EE caught self with EE's right palm, which in turn jammed EE's arm into EE's elbow causing it to snap and dislocate.
Cleaning rock from conveyor at jaw crusher while repairs were being made. Pulled rock by hand at eye level toward them and one hit right front tooth and appears to have cracked the tooth. No pain.
EE was using a cutting torch cutting a piece of steel. Appears that the oxygen line blew out causing severe burns to EE's right arm. ER treated EE for 1st and 2nd degree burns and released EE
Slipped or fell from last step of track of Metso LT105 or a step ladder nearby. Assisted from the ground because EE could not support self on the right leg - right knee replacement in January 2020. EE was unsure if knee gave way or made a misstep from of the steps. There was no debris present. EE was worried about the knee implant. Transported to ER.
EE was using the restroom, experienced a dizzy spell, blacked out, fell, and hurt shoulder. Got up, made the way back to the office nearby, while sitting in chair, EE experienced another dizzy spell and blacked out again. At that time, co-worker called ambulance.
Lost footing on wet platform, slipped and reached for vertical post, hooked arm at bend of elbow. ee stated ee had injury to left arm.
Employee was mounting a Caterpillar 966 loader EQ #23.3814. Apparently there was frost on the loader grab bars and when ee reached with left hand it slid down and ee fell backwards. EE indicated ee must have caught hands and thought left arm was broken.
He was lifting signs that were for stockpile identification to place in the back of a pickup truck. It was raining and he slipped and twisted his back trying not to fall.
Employee reported to me at 7:27 AM that he stepped on a rock and twisted his right ankle. Refused treatment at that time, because he didn't think he hurt it that bad. On 6/20/2014 at 11:05 AM, he stated that his ankle was still sore and hurt. Advised employee to have x-rays immediately and seek treatment. No witnesses to the accident
Employee was assisting a co-worker hook up to a pressure washer trailer. The trailer was not aligned with the hitch on the co-worker's truck. Employee tugged on tongue trailer and had a sharp pain in his left side of rib cage.
EE said he thought that maybe his boot was wet and that caused his left foot to slip while mounting his Euclid Water Truck (26.3604), causing pain to his upper left leg. Injury was not reported until 6:45 AM on November 18, 2013 stating that he needed to be off for a few days until he got better. Co. advised EE to have medical treatment.
Arrived at the job, got out of the truck, became dizzy & passed out - went by ambulance to the hospital.

He was spraying down a screen with a pressure washer. As he was turning around he slipped and with his hand still on the trigger of the pressure washer, it penetrated his right leg from the water and pressure.
Employee was operating an Euclid R35 haul truck. He lost control coming over a haul road and struck a berm. Operator stated he did not have any brakes. He also stated that his brakes did not feel right while he was being loaded. However, he did not notify anyone or park the truck until a mechanic could come look at the truck.
The portable screen got jammed and he was trying to unstop; took a bar to unstop and a rock bounced off and hit his left hand
EE tripped in door trench in back of shop while exiting the building. His knee swelled immediately. He did not want to go to the doctor at that time, but will leave early to ice and elevate the leg, stating he had surgery some years ago on this knee. If not better over the weekend he said he would seek medical attention.
EE was dismounting a Mack dump truck at the end of is shift, as he was stepping down, he said his knee popped and could not walk. Steps were clean of debris and in good condition. He said his knee just popped and nothing with the vehicle or the ground condition had anything to do with it. He stated it was the same knee that had been swollen a few weeks ago. Taken to ER, has sprain.
EE was pressure washing a screen when he slipped and hit his knee on the screen rail. The deck was wet and muddy from the use of the pressure washer.
EE was working on the installation of a vibratory screen, while on a ladder he pulled the aluminum catwalk back in place but was not clear enough to keep from being struck in the head. He was taken to the hospital to be treated and received stitches and was released with no restrictions.
EE was exiting a Volvo A30 to empty a bin when he slipped & fell on truck steps during rainy weather. He hit his elbow against the side of the truck causing bruising and pain to his elbow and shoulder. No other damage to him or the equipment was noted. He informed his supervisor of his injury who arranged for IE to be taken to hospital for treatment.
He was using the pressure washer cleaning his truck and the hose busted and caused the hose to spray his stomach area.
He slipped when he was climbing down from the excavator. He did not fall however, the impact from missing a step was jolting. *Note..this report was originally filed on 7/10/07, because he was complaining of pain but had not made an appt with a Dr till 7/17/07. He was having knee therapy & we instructed him to see a specialist. We sent him home 8-10-07, MRI 8-13-07;surgery 9-13-07
Repairing wiper arm when it trapped his right hand. His hand became bruised with a slight amount of swelling.
A heavy piece of metal, He was going to flip the piece of metal on a table and when he did, he hurt his back. We did not work the next day and he thought it would be okay. However, when he returned to work on Monday, 2-13-06, it was still hurting so we took him to the hospital for x-rays.
He was handing another employee a piece of metal when his foot slipped and he fell approximately 4 to 5 feet landing on his back.
REMOVING A LODGED ROCK UNDER JAW CRUSHER; ANOTHER ROCK FELL; MASHED AND CUT HIS MIDDLE FINGER ON RIGHT HAND.
HAUL TRUCK HAD A WATER LEAK. OPERATOR NOTIFIED LOADER OPERATOR OF BREAKDOWN. TRUCK OPERATOR STEPPED INSIDE OF LEFT FRONT WHEEL TO EXAMINE LEAK. OPERATOR OF LOADER MOVED AND STRUCK BACK OF TRUC K CAUSING TRUCK TO MOVE FORWARD ONTO LEFT FOOT OF HAUL TRUCK OPERATOR.
OPERATOR WAS STEPPING INTO HAULING UNIT AND LOST HIS FOOTING. HE FELL INTO THE DOOR FRAME STRIKING THE LOWER ABDOMINAL REGION.
HE WAS DRIVING DOWN THE HILL AND THE SEAT BROKE, HURTING HIS BACK.
DRIVING STOCKPILE TRUCK, PULLED #3 LEVER BACK, HAND SLIPPED AND THE LEVER STRUCK HIM IN THE GROIN.
FLASHBURN AND SMALL PIECE OF STEEL IMBEDDED IN LEFT EYE, WAS WEARING SAFETY GLASSES.
