Truck was hauling overburden and dumping over the face of the highwall. The berm got extended past the rock. When they started dumping, the material sluffed under the tires and the truck slid 25' down and ended up partially on it's side. The driver broke a thumb and bruised a shoulder and hip.

While removing the bolts from the counterweight of a Hitachi ZX850H, the counterweight fell backwards onto employee's lower right leg, severing it above the ankle.
TEMP WAS 95 DEGREES F, SHIFT WAS NEARLY OVER. EE DROVE 250 YDS THEN SLOWLY TOUCHED THE BERM ON RIGHT SIDE EVENTUALLY CUTTING THROUGH THE BERM THEN ROLLED 1 1/2 TIMES DOWN A 40 FT EMBANKMENT.
POSSIBLY LIFTING ROCKS OR CONTINUOUS USE OF HEAVY EQUIPMENT.
TOUCHED ELCTRICA; BOX WITH SCREWDRIVER AND CAUSED ELECTRICAL FALSH.
