The employee was operating a forklift when a strap on the floorboard became wrapped around the front wheel. The end of the strap caught the employees boot heel and pulled EE out of the forklift. The forklift then ran over EE's ankle, breaking it in 3 places.

EE was moving material got stuck on the side of the hill, after unloading load went hown the hill hit a bump at the bottom and hit his head inside the cab. Was taken to yard by loaderman and 911 was called.

EE SLIPPED AND STUCK HAND INTO FAN BLADES ON MILLER WELDING MACHINE WHILE TRYING TO ADJUST GOVENOR. EE HAD RAISED GUARD ON ENGINE TO GET TO GOVENOR.
