EE was cleaning the mud off of a conveyor roller. He was using a hand help scraper to do so. His scraper came n contact with the convery belt and the handle of the scraper struck him in his left knee. He did go to the doctor and was placed on light duty.

Showing 50 of 80EE was working on replacing wear iron in the Sag Mill. He had his hand on the boom of the lining machine. The boom was retracted and his hand was caught between two of the boom sections. He did go to the doctor and received stitches in his left hand.
Injury was grinding on the cab of track dozer DO-5. When the shop doors were opened the wind blew some metal fines into his right eye. He was taken to the E.R. where the metal was removed.
Employee was operating a Cat D-8 dozer on a ore stockpile. He doesn't know when the injury happened. His back was sore and having spasms on 11-16-08. He went to the doctor and was not allowed to return to work. He contacted his supervisor on 11-21-2008 about his injury.
Employee was climbing up the ladder on a DM45 drill. His hands slipped off the handholds and he fell forward striking his face on the upper part of the ladder. He did go to the E.R. and received 7 stitches.
EE was working in the mill, he was replacing a grease cover on a lube system for the regrind mill. EE extened his arms above his head to place the cover and in doing so he felt a pain in his left arm and shoulder, Went to Dr and was placed on on light duty limiting the use of his left arm.
EE was using a utility knife to cut the locks on a plastic nordbak bucket. While cutting the locks, whe slipped with the knife and cut her right ghigh. She did go to the doctor and received 4 stitches. There were no work restrictions, so she id return to full work duties. She had the stitches removed on 10/15/08.
EE was opening the crusher shack door. His index finger on his right hand had gotten caught between the door and the door jam. He was taken to the hospital were he received three stitches in his finger.
Employee was operating a 16G Motor Grader. She was backing up after making a pass. She was looking over her right shoulder when her neck started to hurt. She initially thought it would be OK and didn't want to go to a Dr. On August 14, 2008 she decided to go to the doctor. She was then placed on light duty.
EE was setting light plants up for the night mine crew. He was attempting to attach Q15 light plant to his pickup, light plant jack was to low so EE attempted tp lift the light plant onto his truck hitch. In doing so he strained his lower back, he went to Dr 7/8/08 and was placed in light duty.
Ee was performing maintencne on a 325 cat excavator. He was replacing a control leverin the cab. He had dropped a tool and bent over to pick it up. When he stood up the operator cab door had swung closed and struck EE in the head. He went to the emergency room where he was released. On 3/18/08 he went back to his dr and was not able
On 03/17/08 EE was walking down the stairway by the welding shop in the mill, he slipped and fell down the stairs. He initially didn't go to the DR and felt fine. By the end of the week his back was still sore, so on 3/21/08 he went to the DR and was placed on light duty work.
Employee was operating a drill press. While operating the drill the glove on his right hand was caught in the rotating drill bit. He sustained a cut on his right hand. He did go to the emergency room and received 6 stitches.
Employee was installing bed liners on a 785 haul truck. He was using a portable 9" grinder. In doing so he had gotten metal in his eye. He wasn't sure if he had metal in his eye or not. So on 11-22-07 I took him to the emergency room where the metal was removed. Employee did go to an eye doctor on 11/23/07 and again on 11/26/07. His eye is o.k. and he did not miss any work.
Employee has an occupational disease. He has carpal tunnel syndrome in his left hand. He will got to the doctor on 11-12-07. Then for his surgery on 11-14-07.
EE had been recovering from an off the job, when he was watching a bumper being repaired on a straight truck when he stepped back to look at the bumper, stepped back, he stepped on a rock, caused him to lose his balance and fall onto his right hand, fracturing his wrist.
A unplanned detonation occurred at the bottom of the pit. A caterpiller D-11 dozer company number B-12 was doing clean up in this area when this occurred. MSHA hotline number was called immediately. There were no injuries in this explosion and no equipment damage.
Mounting a 3300-51 tire on a wheel, pulling the lock ring up onto the tire he got his thumb caught between the lock ring & flange ring while being supervised.
EE was doing a pre-shift inspection on a 235 cut rock breaker. In doing so, he stepped in some mud and his foot slipped on a rock. This caused his knee to pop. EE went to the ER on 5/26/07 and visited his doctor on 5/30/07, where he was told he could not return to work until more tests were done. This is a LTA.
Employee was performing maintenance checks for problems on a 5230 shovel. While working on the shovel a small rock fell from the highwall. The rock rolled under the maintenance truck and struck him in the left leg. He was taken to the hospital where he received 3 stitches in his left leg.
Employee was welding on bed liners of a haul truck. In doing so, hot slag burned his left elbow and it got infected. He went to the doctor on Tuesday the 30th and the doctor decided not to let him return to work until the infection was gone.
Employee was putting a bellypan on a cat dozer. He put his knees to his chest & pushed with his feet. In doing so he felt a pop in his back on the right side of his ribs. He continued to work his shift & also his next shift before going to the hospital on 12/9/06. He then went back to the clinic on 12/13/06 & was put on light duty.
Employee was marking out drill pattern & stepped on a loose rock, twisting his knee. He stated it was an old injury he hand gotten while drilling in Colorado. He decided to go to the Dr., nothing was found wrong. He went back & he was scheduled for an MRI on 12/1/06. Nothing was found wrong again but the Dr. decided to fit him with a leg brace.
EE BACKED HIS HAUL TRUCK UP TO A BERM TO DUMP HIS LOAD. THE LOAD LEFT THE BOX AND HELD THE BOX IN THE RAISED POSITION. EE PUSHED THE LOWERING LEVER DOWN AND PULLED AWAY FROM THE BERM. THE RAMS BEGAN LOWERING AND THE BOX DROPPED SEVERAL FEET, JARRING EE'S NECK. CHIROPRACTOR & THERAPY & DAYS OFF.
EE IS A SURVEY TECH. & WAS ASSISTING MOVING EMPTY PALLETS, CLEANING A LAY DOWN AREA. HE FELT A STRANGE SENSTAION IN HIS LOWER RIGHT ABDOMEN. AFTER SOME TIME & SEVERAL Dr. VISITS, HE WAS DIAGNOSED WITH A MINOR RUPTURE THAT HE MAY NEED SURGERY FOR. NO TIME MISSED FROM WORK.
EE WAS TRAVELING INTO THE MINE WHEN A ROCK APPROX 6' X 6' X 8' ROLLED DOWN AN ANGLE OF REPOSE, JUMPED A SAFETY BERM AND STRUCK THE SIDE OF HIS 992 LOADER. EE WAS THROWN OUT OF THE CAB AND RECEIVED A CUT ON TOP OF HIS HEAD AND ON HIS LEFT ELBOW.
EE WAS ATTEMPTING TO LIFT A TORQUE CONVERTER THAT WAS PARTIALLY BOLTED INTO A 992 LOADER. THE PICKING EYES BROKE CAUSING THE CRANE BLOCK TO JUMP SEVERAL FEET IN THE AIR AND ONE OF THE CABLES TO COME PARTIALLY OFF THE SHIV. HE DID NOT NOTICE THE FAULTY SITUATION & CON'T HIS JOB ABOUT 3' UNDER THE 200# BLOCK. THE BLOCK DROPPED ONTO HIS HARD HAT PINNING HIM AGAINST THE LOADER.
EE WAS CLIMBING ON TO A TRACK MOUNTED ROCK BREAKER FOR THE FUELING/LUBE PROCESS AND STRUCK THE TOP OF HIS LEFT FOOT ON THE BOTTOM OF THE LADDER RUNG. X-RAY AT THE TIME SHOWED NOTHING BROKEN AND ALL OK. ON 8/26/05 HE WENT TO A CHIROPRACTOR FOR AN ADJUSTMENT ON FOOT. EE was diagnosed with a broken foot on 9-21-05. He had surgery. Dr released him to lite duty on 9-30-05.
EE COMPLAINED OF BACK PAIN ON 7/24/05 FROM THE SEAT IN HIS HAUL TRUCK BOTTOMING SEVERAL TIMES DURING THE SHIFT. HE CALLED OFF FOLLOWING SHIFT BUT GAVE NO REASON. INJURY DATE 7/22/05. THIS SEAT WAS IN THE TRUCK FOR 3 WEEKS AND WAS CHANGED OUT AFTER HIS INJURY. MANUFACTURER FOUND NO REASON FOR IT TO BE BOTTOMING OUT
EMPLOYEE & HIS PARTNER WERE LIFTING A WOODEN TIMBER, ONE MAN ON EACH END OF THE TIMBER. THE TIMBER WAS 12"X 12"X 6'LONG, WEIGHING APPROX. 175 LBS. EMPLOYEE USED HIS LEGS TO PICK THE TIMBER BUT THEN TWISTED WITH THE LOAD. ORTHOPEDIC SURGEON SAID NO WORK UNTIL FURTHER NOTICE.
Employee was walking from one pickup truck to another. The ground was frozen and rutted. She rolled her ankle and felt a sharp pain. She was seen at the emergency room and diagnosed with a small fracture to the ankle.
THE EE WAS CARRYING A SET OF WELDING LEADS AND STEPPED FROM ONE LEVEL OF GRATED DECKING DOWN TO THE NEXT LEVEL - APPROX. 10 INCHES DOWN. EE STEPPED ONTO A 2.5"-DIAMETER STONE AND FELT A SHARP PAIN IN HIS LEFT FOOT. X-RAYS AT THE ER SHOWED A FRACTURED BONE IN HIS LEFT FOOT. DOCTOR PRESCRIBED LIGHT DUTY.
EE WAS CROSSING UNDER A 5'6" STEEL FRAMEWORK AND BUMPED HIS HARD HAT, JAMMING HIS NECK. ON 3/28/05 EE INFORMED HIS SUPERVISOR THAT HE HAD SEEN A CHIROPRACTOR FOR AN "ADJUSTMENT."
EE AND TWO OTHER MILL EMPLOYEES WERE DRAGGING A 70'X2" RUBBER WATER WASH DOWN HOSE INTO POSITION TO WASH THE MILL FLOOT. EE FELT A SHARP PAIN IN LEFT CALF. EE WAS DIAGNOSED WITH A STRAINED ACHILLES TENDON AND WILL REQUIRE THERAPY. RETURN TO MOD. DUTY.
TWO MAINTENANCE EE WERE REMOVING 992 LOADER BUCKET HEEL PLATE. ONE EE WAS KNOCKING OUT A BOLT THAT HAD BEEN TORCHED OFF. OTHER EE WAS TORCHING THE ONLY OTHER BOLT HOLDING THE PLATE. THE PLATE FELL APPROX 4.5 FT ONTO EE FOOT, JUST ABOVE STEEL TOE. BROKEN BONE IN FOOT
EE WAS ATTEMPTING TO INSERT 8" PLASTIC PIPE INTO THE PIPE FUSER. AS THE PIPE SLID FORWARD, EE GOT RIGHT LITTLE FINGER BETWEEN THE PIPE AND THE FUSER. FINGER WAS LACERATED REQUIRING STITCHES AT THE EMERGENCY ROOM
EMPLOYEE WAS USING A WIRE FEED WELDER WHEN THE STRINGER SLIPPED OUT OF HIS RT.HAND. HE ATTEMPTED TO GRAB IT W/HIS LEFT HAND & THE WIRE PIERCED HIS GLOVE & HAND. THE WIRE STABBED HIS INDEX FINGER TENDON CAUSING AN INFECTION. DR. NEEDED TO CUT & DRAIN THE INFECTION.
TWO EMPLOYEES WERE DISLODGING A JACK PAD FROM THE MUD ON A DRILL PATTERN. THE IR DRILL HAD LOST ITS PAD EARLIER THAT MORNING. THE SUPERVISOR HAD ADVISED THEM TO USE AN EXCAVATOR TO DO THE JOB IF NECESSARY. THEY USED A PRY BAR AND TOGETHER CARRIED THE PAD TO THE DRILL. LATER THAT SHIFT, ONE EMPLOYEE COMPLAINED OF NECK AND UPPER BACK PAIN.
Ee was drilling a horizontal well, working alone. He changed many drill steels during the shift and at the end of the shift, his left lower leg was very sore. He filed an injury report with the company on 9/7 and on 9/14 he went to a doctor as the pain did not subside. Diagnosed torn tendon on 9/14/04.
MINE MECHANIC WAS USING A HAND HELD PNEUMATIC DIE GRINDER TO CUT A PIECE OF METAL ON A HAUL TRUCK ENGINE. THE 3 INCH GRINDING WHEEL BROKE AND A PIECE OF IT STRUCK HIM ON THE LOWER LIP. THREE STITCHES WERE PLACED IN HIS LIP TO CLOSE THE LACERATION.
NO ACCIDENT OCCURRED. EE IS EXPERIENCING PAIN IN HIS THUMB AND HAS BEEN DIAGNOSED WITH "TRIGGER FINGER". THIS IS AN INDUSTRIAL DISEASE SIMILAR TO CARPAL TUNNEL SYNDROME.
EE WAS CARRYING A PIECE OF 8" DIA PLASTIC PIPE AND DROPPED IT ON HIS FOOT. FURTHER MEDICAL REVIEW FOUND THAT THE BIG TOE WAS BROKEN. EE IS NORMALLY A TRUCK DRIVER BUT WORKED AN OVERTIME SHIFT IN PIT DEWATERING. NO LOST WORK TIME.
EE BEGAN HAVING NUMBNESS AND TINGLING IN HIS NECK, SHOULDER AND RT ARM AND RT HAND. HE SAW A DR ON 1-15-04 AND WAS DIAGNOSED WITH PROBLEMS FROM HOLDING A TELEPHONE TO HIS EAR. NO MISSED WORK.
Employee began to feel pain in his rt. knee as he was operating the cat loader. The pain did not subside and he went to the Dr., was diagnosed with torn cartilage. No accident occurred, he is scheduled for surgery sometime in March. Workers Comp. accepted claim on 2/23/04.
EE EXPERIENCED A PAIN BETWEEN HER SHOULDER BLADES AS SHE WAS DRIVING HER LOADED HAUL TRUCK TO THE WASTE STOCKPILE. HER FINGERS OF LEFT HAND BEGAN TO TINGLE. EMERGENCY ROOM DOCTOR DIAGNOSED STR AINED MUSCLES IN UPPER BACK AND SHE SAW A CHIROPRACTOR FOR AN "ADJUSTMENT" ON 11-17-03.
EE WAS ATTEMPTING TO ROLL A MILL LIFTER (200#) ONTO A LIFTING SLING AND ROLLED IT ONTO HIS FINGER, SMASHING THE FINGER TIP BETWEEN THE LIFTER AND THE DECK. STITCHES WERE NEEDED TO CLOSE THE WO UND.
EE WAS USING A 12# SLEDGE HAMMER AS A BACKING PLATE WHILE ANOTHER EE WAS STRIKING A PIN INTO THE PAN FEEDER DRIVE CHAIN. A SMALL SHARD OF METAL SPLINTERED OFF THE PIN AND STRUCK EE IN THE CHIN REQUIRING STITCHES AND REMOVAL OF THE 1/4" METAL CHIP.
IMPROPER RIGGING OF 70# BEARING CAPS; FAILURE TO WAIT FOR SIGNALS; MISJUDGEMENT OF PROPER CLEARANCE; FAILURE TO CLEAR PERSONNEL BELOW. VICTIM FELT HE WAS IN A SECURE POSITION AND FAILED TO CON NECT TO PROVIDED SAFETY BLOCK. RESULTED IN DISLOCATED RIGHT ELBOW, SCALP LACERATION, POSSIBLE RIB FRACTURES, ABRASIONS AND CONTUSIONS. HOSPITALIZED.
THERE WAS NO SPECIFIC ACCIDENT OR INCIDENT BUT MR. SATRE HAS BEEN EXPERIENCING LOWER BACK PAIN AND RT LEG & RT HIP PAIN FOR APPROXIMATELY THE PAST MONTH. HE BEGAN CHIROPRACTIC TREATMENTS ON 5-22-03 WORKERS COMP HAS ACCEPTED THIS CLAIM AS INDIVIDUAL DISEASE CLAIM DUE TO NO SPECIFIC ACCIDENT.
EE WAS ATTEMPTING TO OPEN A SMALL CABINET THAT WAS MOUNTED ON THE DECK OF A FLATBED SERVICE TRUCK. THE LATCH WAS JAMMED SO HE BENT DOWN TO GET A CLOSER LOOK AND JERKED ON THE DOOR. THE 12" X 24" DOOR WHICH IS HINGED AT THE BOTTOM OPENED AND STRUCK HIM ON THE LIP AND SLIGHTLY CHIPPED A FRONT TOOTH.
