Employee was spraying off concrete pad under Jaw Crusher. He said he slipped and sprained his ankle. He said he didn't need to go to the doctor, he just went on home. On 12/01/13 he reported to work and worked for several hours, then said his ankle was still bothering him, so he went to doctor the next day.

EE was using a water hose to spray off a concrete pad. He bent over to place the water hose on the ground and as he stood up he felt a pain in his lower back.

Employee was using a water hose to wash a concrete pad. He stepped off the concrete pad (3" elevation change) and lost his balance. He fell to the ground and struck his hand on a sharp rock causing a small laceration which required stitches.

Employee was attempting to adjust a dozer's track roller by striking it with a sledge hammer when a piece of metal from the roller struck him in the lower torso. The injury required 3 stitches.

EE told a supervisor he felt a pull in groin. The details didn't appear to be something that could be work related so the supervisor told him he could leave. The ee told the supervisor later that he had pulled something in his groin while moving a screen. Information obtained from the doctor indicates this could be a recordable inj due to conflicting MD information.

Miner was climbing the ladder to his truck and turned to look at another miner. As he turned, he felt a "pinch" in his back. The injury was first reported by the miner as a NON-WORK RELATED INJURY. On 10/12/10, the miner claimed the injury as work related to the Safety Director.

Employee was walking beside a handrail and felt a stinging sensation on his finger. He looked at the finger and noticed blood dripping from a finger. He had apparently hit a metal weld burr and cut his finger.
