Running in pit and tripped. Sprained right ankle.
Employee was cutting a conveyor belt with a box cutter. The blade slipped and punctured leg just above knee. Employee went and got three stitches and was back to work the same day.
After Locking out the machine, the foreman was inside working on replacing wear pads. Upon completion of his job EE came over to see what the foreman wanted done. The foreman was exiting the machine which entailed turning the rotor. When turning the rotor it caught EE's finger between the rotor and side plates.
Slipped and hit his head on some iron on the back of his head.
A service mechanic was repairing a screen plant when he reached around to hold the head of a carriage bolt with his gloved hand. The air wrench on the other end spun the bolt tearing the mechanic's glove and cutting the left middle finger. The injury required 5 stitches and a band aid.

OVERHEAD ELECTRICAL POWER LINE ARCED TO CRANE BOOM. EE WAS HOLDING CABLE WIRES AND WAS SHOCKED.
