MINER WAS HAND 4UCKING ORE OFF OF A BENCH, WITH NO PRIOR WARNING THE BACK MADE A LOUD POP AS THE SLAB BROKE LOOSE AND FELL, STRICKING HIM ON THE HEAD AND LEFT SHOULDER. COMING TO REST ON HIS CHEST AND LEGS.3

EVIDENCE INDICATES AN ACCIDENT OCCURRED ON OCT. 6,1987. A MINER WAS INJURED WHEN A FALL OF GROUND OCCURRED. THE INJURIES WERE A FRACTURE PELVIS AND A FRACTURED R. FIBULA. THE MINER SUFFERED LO ST TIME FROM 10-6-87 UNTIL 11/30/87. THE OPERATOR FAILED TO REPORT THE ACCIDENT TO MSHA ON FORM 7000-1. THE ACCIDENT SHOULD HAVE BEEN REPORTED WITHIN TEN WORKING DAY AFTER THE INJURED OCCURRE
